摘  要:目的:探讨慢性阻塞性肺疾病(COPD)D组家庭无创机械通气治疗效果。方法:选取重症医学科治疗后处于稳定期且符合2011年COPD D组患者31例,其中治疗组15例给予家庭无创机械通气及常规治疗,对照组21例仅给予常规治疗。随访1年,观察两组治疗后COPD急性加重发生频率、总住院时间、COPD评估测试(CAT)评分、FEV1占预计值百分比、PaO_2、PaCO_2、气管插管率、死亡率变化。结果:治疗组与对照组1年内急性加重次数分别为(3.67±0.72)次和(4.48±1.03)次,差异有统计学意义(P<0.05);1年内总住院时间分别为(67.1±36.5)d和(91.0±30.0)d,差异有统计学意义(P<0.05);气管插管分别为1例和2例,差异无统计学意义(P>0.05);死亡分别为1例和1例,差异无统计学意义(P>0.05);1年后CAT评分分别为(26.0±3.5)和(30.6±3.8)(P<0.05);FEV1占预计值百分比(33.3±5.1)和(28.7±4.5),差异有统计学意义(P<0.05);PaO_2分别为(62.2±4.7)和(55.6±3.7)mmHg,差异有统计学意义(P<0.05);PaCO_2分别为(60.5±5.9)和(68.8±5.6)mmHg,差异有统计学意义(P<0.05)。结论:COPD D组家庭无创机械通气治疗能减少急性发作,减少住院时间,增加FEV1占预计值%,改善CAT评分,增加PaO_2、降低PaCO_2水平。Objective To investigate the effect of household non-invasive mechanical ventilation therapy of patients group D with chronic obstructive pulmonary disease(COPD). Method 31 patients in the COPD D group were in a stable period after the treatment. Among them,15 patients in the treatment group were given non-invasive mechanical ventilation and routine treatment,while 21 patients in the control group were only given the routine treatment. The frequency of exacerbations,total length of hospital stay,COPD assessment test(CAT),FEV1 to the predicated value,PO2,PCO2,trachea cannula rate,mortality change of two groups after one year follow-up visit were observed. Results The observation results of the treatment group and control group were as follows,the frequency of exacerbations during one-year time was(3. 67 ± 0. 72) and(4. 48 ± 1. 03) respectively(P〈0. 05); the total length of hospital stay was(67. 1 ± 36. 5) days and(91. 0 ± 30. 0) days respectively(P〈0. 05); mortality was one vs one(P〈0. 05); CAT was(26. 0 ± 3. 5) and(30. 6 ± 3. 8) respectively(P〈0. 05); FEV1 to the predicated value was(33. 3 ± 5. 1) and(28. 7 ± 4. 5) respectively(P〈0. 05); PO2 was(62. 2 ± 4. 7) mm Hg and(55. 6 ± 3. 7) mmHg respectively(P〈0. 05); PCO2 was(60. 5 ± 5. 9) mmHg and(68. 8 ± 5. 6) mmHg respectively(P〈0. 05). Conclusion The household non-invasive mechanical ventilation therapy of COPD group D can reduce the incidence of acute exacerbation,the hospitalization time and PCO2 level,and can increase the FEV1 to the predicted value,the CAT score and the PO2.
