龙胆泻肝汤治疗带状疱疹的效果及对p38 MAPK基因表达的影响

摘　　要：目的分析龙胆泻肝汤治疗带状疱疹的临床疗效及对患处皮肤p38 MAPK基因表达的影响。方法选取2016年1月~2017年6月广州市红十字会医院皮肤科收治的带状疱疹患者153例,采用随机信封法分为观察组(80例)和对照组(73例)。对照组采用基础治疗联合红蓝光治疗,观察组在对照组基础上联合龙胆泻肝汤治疗。记录两组患者术后水疱消退时间、结痂时间及疼痛缓解时间,并评估临床疗效;检测治疗前及治疗结束后4周患者皮肤中p38 MAPK基因表达水平。结果治疗后,观察组患者水疱消退时间、结痂时间及疼痛缓解时间均显著短于对照组(P<0.05);治疗后,观察组患者治疗总有效率显著高于对照组(P<0.05);治疗后,两组患者患处皮肤p38 MAPK基因相对表达量均显著降低(P<0.05),且观察组显著低于对照组,差异有统计学意义(P<0.05)。结论龙胆泻肝汤联合红蓝光治疗带状疱疹可有效提高临床疗效,改善病灶区p38 MAPK基因水平。Objective To analyze the clinical efficacy of Longdan Xiegan Decoction in the treatment of herpes zoster and its influence on the expression of p38 MAPK gene in the affected skin. Methods Clinical data of 153 cases treated in the Dermatology Department of Guangzhou Red Cross Hospital from June 2017 to January 2016 was randomized into observation group（80 cases） and control group（73 cases）. The control group received basic treatment combined with redblue light treatment, the observation group was treated with Longdan Xiegan Decoction combined with red-blue light therapy. Both groups were treated for 4 weeks. Blister subsided time, scabby time, pain relief time were recorded before and after treatment, the clinical curative effect was evaluated. At the same time, the expression level of p38 MAPK gene was detected in each group. Results After treatment, blister subsided time, scabby time, pain relief time in the observation group were significantly shorter than those of the control group（P 〈 0.05）, the total effective rate of the observation group after treatment was significantly higher than that of the control group（P 〈 0.05）. In the two groups, the expression levels of p38 MAPK after treatment were significantly lower than those before treatment（P 〈 0.05）, while the observation group was significantly lower than that of control group（P 〈 0.05）. Conclusion Longdan Xiegan Decoction combined with red-blue light in the treatment of herpes zoster can effectively improve the clinical efficacy and the level of p38 MAPK gene in the attacked area.
