摘　　要：目的探讨不同术式对胆总管结石治疗的疗效。方法选取我院2010年2月至2017年5月收治的100例胆总管结石患者,随机分为T组(56例)和C组(44例)。T组给予腹腔镜下胆总管切开探查取石术(LCBDE),C组采用传统开腹胆总管探查取石术+T管引流术。比较两组手术效果。结果两组患者手术时间、术后24 h血淀粉酶水平、术后14 d肝功能指标方面差异无统计学意义(P>0.05);T组的术中出血量明显少于C组(P<0.05);T组术后胃肠道功能恢复时间、住院时间均明显短于C组,差异均具有统计学意义(P<0.05);T组并发症发生率明显低于C组(P<0.05)。结论 LCBDE治疗胆总管结石的疗效好且安全、并发症少,是值得临床推广应用的一项微创技术。Objective To explore the therapeutic effects of different surgical methods on choledocholithiasis. Methods From February 2010 to May 2017, 100 cases of choledocholithiasis in our hospital were selected and randomly divided into group T（56 cases） and group C（44 cases）. The patients in the group T were given laparoscopic common bile duct explo-ration by the same surgeon, while traditional exploratory choledochotomy and T-tube drainage were applied to patients in the group C. The results of the two groups were compared. Results There was no statistical significance in the two groups of patients with operation time, serum amylase level of 1 d after operation and liver function index of 14 d after operation（ P〉0.05）. The intraoperative blood loss of the group T was significantly less than that of the group C（P〈0.05）. The recovery time of gastrointestinal function after operation and hospitalization time in the group T were significantly shorter than those in the group C, the differences had statistical significance（P〈0.05）. The incidence rate of complications of the group T was significantly lower than that of the group C（P〈0.05）. Conclusion Laparoscopic choledocholithotomy is a safe and minimally invasive procedure for choledocholithiasis. It is a minimally invasive technique which is worthy of clinical application.
