负压引流联合股前外侧皮瓣移植用于修复四肢大面积软组织缺损的临床效果

摘　　要：目的探究封闭式负压引流(VSD)联合股前外侧皮瓣移植治疗四肢大面积软组织缺损的疗效。方法 32例四肢大面积软组织缺损患者,根据治疗方法不同分为对照组(17例)和观察组(15例)。对照组接受股前外侧皮瓣移植修复术,观察组接受股前外侧皮瓣移植修复术联合VSD术,观察比较两组的治疗效果、皮瓣愈合时间、住院时间以及皮瓣移植后相关并发症。结果观察组患者的总有效率100.00%明显高于对照组的76.47%,差异具有统计学意义(P<0.05)。观察组的皮瓣愈合时间(10.32±1.45)d和住院时间(18.32±2.54)d均短于对照组的(16.54±2.11)、(22.17±3.04)d,差异具有统计学意义(P<0.05)。观察组患者的皮瓣移植后相关并发症发生率6.67%低于对照组的41.18%,差异具有统计学意义(P<0.05)。结论四肢大面积软组织修复治疗中VSD与股前外侧皮瓣移植联合应用有助于促进皮瓣的成活愈合,缩短患者的住院时间,值得推广。Objective To investigate the efficacy of vacuum sealing drainage （VSD）combined with anterolateral thigh flap transplantation for repairing large area soft tissue defects of extremities. Methods A total of 32 patients with large area soft tissue defects of extremities were divided by different treatment Methods into control group （17 cases） and observation group （15 cases）. The control group received anterolateral thigh flap transplantation, and the observation group received anterolateral thigh flap transplantation combined with VSD. Observation and comparison were made on treatment effect, flap healing time, hospitalization time and related complications after flap transplantation between the two groups. Results The observation group had obviously higher total effective rate as 100.00% than 76.47% in the control group, and the difference was statistically significant （P〈0.05）. The observation group had shorter flap healing time as （10.32±1.45） d and hospitalization time as （18.32±2.54） d than （16.54±2.11） and （22.17±3.04） d in the control group, and the difference was statistically significant （P〈0.05）. The observation group had lower incidence of related complications after flap transplantation as 6.67% than 41.18% in the control group, and the difference was statistically significant （P〈0.05）. Conclusion Combined application of VSD and anterolateral thigh flap transplantation in the repair of large area soft tissue of extremities helps to promote the survival of the skin flap and shorten the patient’s hospitalization time. It is worth promoting.
