椎管内麻醉分娩镇痛对产程及母儿影响的临床观察

摘　　要：目的观察分娩镇痛对产程及母儿结局的影响。方法选取100例行椎管内麻醉分娩镇痛产妇作为观察组,另选同期100例未行镇痛产妇作为对照组,观察比较两组产妇镇痛分娩结局(胎儿窘迫、剖宫产)及活跃期至分娩时长、使用宫缩素情况。结果观察组患者的胎儿窘迫及剖宫产发生率分别为16%和14%,与对照组的15%和19%比较差异无统计学意义(P>0.05)。观察组的平均分娩时长(3.58±0.25)h短于对照组的(4.87±0.32)h,差异具有统计学意义(P<0.05)。两组使用缩宫素情况比较差异无统计学意义(P>0.05)。结论椎管内麻醉分娩镇痛与胎儿窘迫无直接关系,未增加剖宫产率,同时对产程亦无影响。Objective To observe the effect of intraspinal anesthesia for labor analgesia on the birth process and the mothers and children. Methods There were 100 puerpera received intraspinal anesthesia for labor analgesia as observation group, and another concurrent 100 puerpera received no intraspinal anesthesiafor labor analgesia as control group. Observation and comparison were made on analgesic delivery outcome （fetal distress, cesarean section） and duration of active period to delivery and oxytocin usage between the two groups. Results The observation group had no statistically significant difference in incidence of fetal distress and cesarean section as 16% and 14%, comparing with 15% and 19% in the control group （P〉0.05）. The observation group had shorter duration of average delivery as （3.58±0.25） h than （4.87±0.32） h in the control group, and the difference was statistically significant （P〈0.05）. Both groups had no statistically significant difference in oxytocin usage （P〉0.05）. Conclusion Intraspinal anesthesia labor analgesia is not directly related to fetal distress. It will not increase cesarean section rate, and shows no effect on labor.
