摘  要:目的探讨根据腹部横截面积调节管电压技术对CTU图像质量和辐射剂量的影响。方法对临床拟诊为泌尿系统疾病行CTU检查的患者89例,根据平第三腰椎横突水平腹部横截面积分为A组(大横截面积组):横截面积大于等于600 cm^2(30例);B组(中等横截面积组):介于400~600 cm2(30例);C组(小横截面积组):横截面积小于400 cm2(29例)。A、B、C三组排泄期图像管电压分别采用120、100和80 kV进行扫描,对扫描图像进行主观及客观评价,记录三组不同扫描方案的剂量长度乘积(DLP)和CT容积剂量指数(CTDI_(vol)),并计算有效辐射剂量(ED)。比较各组辐射剂量及图像质量的差异。结果 A、B、C三组有效辐射剂量(ED)分别为(5.82±0.58),(3.01±1.29)和(2.28±0.35)m Sv,差异有统计学意义(F=144.78,P<0.01)。Kappa一致性检验示A、B、C三组Kappa值均>0.70,两名医师对图像评价的一致性较高,A、B、C三组比较图像质量评分差异无统计学意义(H=1.58,P>0.05),各组的图像对比噪声比差异无统计学意义(F=0.25,P>0.05)。结论在CTU检查中,根据腹部横截面积调节管电压技术可以明显降低受检者的辐射剂量,而不影响图像的诊断质量。Objective To explore the feasibility of low-dose 320-slice CT urography( MDCTU) based on abdominal cross section area. Methods 89 consecutive patients with suspected urologic diseases were randomly divided into 3 groups of large(S ≧600 cm^2, n=30), medium(400 cm^2≤S600 〈cm^2, n=30) or small(S400 cm^2, n=29) areas according to abdominal cross sectional areas. MDCTU was acquired and image quality was qualitatively compared at 120, 100, 80 k V, respectively for the large, medium, and small cross sectional areas. The dose-length product(DLP) and CT dose index volume(CTDIvol) of the 3 groups were recorded and effective radiation dose(ED) were calculated and compared. Results The difference in the EDs was significant different( F=144.78,P〈0.01) among the large(5.82 ±0.58 mSv), medium(3.01 ±1.29 m Sv), and small(2.28 ±0.35 mSv) cross sectional areas. The image quality scores determined by two physicians were highly consistent with each other(all κ〉0.70). There was no significant difference in image quality(H=1.58, P〉0.05) and contrast to noise ratios among the 3 groups(F=0.25, P〉0.05). Conclusion Abdominal cross-sectional area adjusted tube voltage technique in MDCTU can effectively reduce the effective radiation without compromising image quality.
