摘 要:目的探讨冠心病合并风湿性疾病患者与冠心病不合并风湿性疾病患者在传统风险因素、病变特征,特别是血栓发生风险方面的差异,分析相关临床指标的特点。方法选取2013-2017年在我院住院的冠心病合并风湿性疾病患者49例为研究组,及同期住院的年龄、性别匹配的冠心病不合并风湿性疾病患者的124例对照,比较两组患者的人口特征及传统心血管风险因素,凝血因素等方面的差异。采用Logistic回归方法分析风湿系统疾病对患者凝血功能的影响。结果冠心病合并风湿性疾病患者在性别、D二聚体水平间的差异有统计学意义。女性冠心病患者合并风湿病的风险增高。冠心病合并风湿性疾病患者的D二聚体水平显著升高。在D二聚体ROC曲线分析中,D二聚体的cutoff值为392 ug/L时,约登指数最大,为0.303。诊断的灵敏度为0.898,特异度为0.405。结论与未合并风湿系统疾病的冠心病患者相比,冠心病合并风湿性疾病患者中女性比例增加且D二聚体水平显著升高,需要警惕血栓事件的发生。Objective To investigate the the clinical differences between patients with coronaryheart disease complicated by rheumatic disease arid the patients with coronary heart disease and no rheumatic disease in traditional risk factors and lesion characteristics, specialties the differences in thrombosis occus, we will analyze the characteristics of clinical indicators. Methods We enrolled 49 patients with coronary heart disease by rheumatic disease in Guangzhou medical university affiliated first hospital from from 2013 to 2017 as the research group and another 124 patinets with coronary heart disease and no rheumatic disease who were hospitalized in the same period as the control group. The two groups were matched in age and gender. The two groups we compared by demographic characteristics, traditional cardiovascular risk factors, and Blood coagulation factors. Logistic regression analysis was conducted to examine the influence of rheumatic disease on the incidence of Blood coagulation function. Results the differences between patients with coronary heart disease complicated by rheumatic disease in age and gender was Statistically significant., women with coronary heart disease had a higher risk of rheumatic disease. Compared patients with coronary heart disease, patients with coronary heart disease combined with rheumatic disease had significantly higher levels of D-dimer. In the analysis of the ROC curve of D-dimer, the cutoff value of D- dimer was 392 ug/L, and the approximate index was 0. 303. At this point, the diagnostic sensitivity was 0. 898 and the specificity was 0. 405. Coclusion compared with coronary heart disease patients without rheumatic disease, the proportion of women with coronary heart disease combined with rheumatic disease increased and the level of D dimer was significantly higher, and the attention of thrombosis was necessary.
