半量与全量甘露醇对高血压性脑出血早期血肿扩大影响的Meta分析

摘　　要：目的系统性评价半量甘露醇与全量甘露醇对高血压性脑出血早期血肿扩大的影响。方法按Cochrane协作网系统评价法进行评价,在严格进行文献质量评价的基础上使用Rev Man5.3软件对半量甘露醇与全量甘露醇对高血压性脑出血早期血肿扩大的影响进行Meta分析。结果最终纳入6篇文献包括594例病例,Meta分析结果显示,脑出血24 h内使用半量甘露醇较全量甘露醇可明显降低血肿扩大率[RR=0.38,95%CI(0.21,0.72),P<0.01]。结论脑出血发病24 h内使用半量甘露醇可较好地降低血肿扩大率。Objective To evaluate the influence of half-quantity and full-quantity of mannitol on the early hypertensive cerebellar hemorrhage. Methods Evaluated the researches according to the Cochrane Collaboration and conducted the Meta-analysis with the software Rev Man5. 3 on the basis of strict evaluation of the quality of literature. Results 6 RCTs including 594 cases meeting the criteria were conducted Meta-analysis showed that half-quantity of manitol was better than full-quantity mannitol in lowering the rate of enlargement hematoma [RR = 0. 38, 95% CI（ 0. 21, 0. 72）, P 〈 0. 01 ]. Conclusion In 24 h of hypertensive cerebellar hemorrhage,half-quantity of mannitol can effectively reduce rate of enlargement hematoma.
