Treatment of early osteonecrosis of the femoral head by high energy extracorporeal shock wave combined with Yuanshi Shengmai Chenggu Tablets:a report of 34 cases

摘  要:目的探讨高能量体外冲击波(ESWT)配合袁氏生脉成骨片治疗早期股骨头坏死(ONFH)的临床疗效。方法收集2016年1月至2016年12月期间在广州中医药大学第一附属医院三骨科保髋病区就诊的早期ONFH患者68例,根据随机数表法分为对照组和观察组,每组34例。对照组给予单纯高能体外冲击波治疗,观察组在对照组治疗基础上联合袁氏生脉成骨片治疗,疗程为6个月。治疗后,评价两组患者的临床疗效及ONFH范围,同时比较治疗前、治疗后3个月和12个月时两组患者的VAS疼痛评分和Harris评分。结果观察组患者的治疗总有效率为97.1%,明显优于对照组的76.5%,差异有显著统计学意义(P<0.01);观察组患者的股骨头总坏死率为17.6%,明显低于对照组的61.8%,差异有显著统计学意义(P<0.01);治疗后12个月,观察组患者的VAS和Harris评分分别为(0.23±0.65)分、(88.61±5.34)分,与对照组的(3.53±1.48)分、(79.43±3.62)分比较,差异均有显著统计学意义(P<0.01)。结论 ESWT联合袁氏生脉成骨片治疗早期ONFH比单一ESWT疗效更显著,联合治疗能加速骨细胞生成,降低股骨头塌陷风险,促进ONFH的愈合,达到改善髋关节功能的作用。Objective To investigate the clinical effect of high energy extracorporeal shock wave combined with Yuanshi Shengmai Chenggu Tablets in the treatment of early osteonecrosis of the femoral head(ONFH). Methods A total of 68 patients with early ONFH, who admitted to Third Department of Orthopedics of the First Affiliated Hospital of Guangzhou University of Chinese Medicine from January 2016 to December 2016, were selected and divided into the control group(n=34) and the observation group(n=34) according to the random number table method. The control group was treated with high-energy extracorporeal shock wave therapy, and the observation group was treated with Yuanshi Shengmai Chenggu Tablets. The course of treatment was 6 months. After the treatment, the clinical efficacy and ONFH of the two groups was evaluated, and VAS pain scores and Harris scores before and after treatment for 3 and 12 months were also compared. Results The total effective rate of ONFH in the observation group was 97.1%, which was significantly better than 76.5% of the control group(P〈0.01). The total necrosis rate of ONFH in the observation group was17.6%, which was significantly lower than 61.8% in the control group(P〈0.01). After 12 months treatment, the scores of VAS and Harris were(0.23±0.65) and(88.61±5.34) in the observation group versus(3.53±1.48) and(79.43±3.62) in the control group(P〈0.01). Conclusion The combination of high energy extracorporeal shock wave and Yuanshi Shengmai Chenggu Tablets has more significant effect than the single high energy extracorporeal shock wave in the treatment of early ONFH, which can accelerate bone cell formation, reduce the risk of collapse of the femoral head, promote ONFH healing, and improve hip function.
