摘 要:目的探究血清降钙素原联合血气分析检测对慢性阻塞性肺疾病急性加重期患者临床风险预测的意义。方法前瞻性选取2016年9月到2017年9月来首都医科大学附属北京友谊医院就诊的慢性阻塞性肺疾病急性加重期患者共50例作为观察组,选取同一时期来首都医科大学附属北京友谊医院体检的健康人群共50例作为对照组。对两组患者均行血清降钙素原联合血气分析检测,比较两组患者血清降钙素原和动脉血气分析数值差异。结果对照组和观察组患者p H值、血氧饱和度指标差异无统计学意义(P>0.05),其余指标均存在显著变化(P<0.05)。与对照组患者相比,观察组患者血清降钙素原(0.099±0.041 ng/ml)、二氧化碳分压(77.842±25.763 mm Hg)数值显著升高,而氧分压(51.258±21.159 mm Hg)以及实际碳酸根离子(2.537±1.126 mmol/L)数值显著降低。结论血清降钙素原联合血气分析检测对于慢性阻塞性肺疾病急性加重期患者临床风险预测具有较高的应用价值,有助于进行慢性阻塞性肺疾病的诊断,值得在临床进行推广应用。Objective To investigate the significance of serum procalcitonin combined with blood gas analysis in predicting the clinical risk in patients with acute exacerbation of chronic obstructive pulmonary disease patients. Methods A total of 50 patients with acute exacerbation of chronic obstructive pulmonary disease from September 2016 to September 2017 were selected as the observation group prospective. The same period of the normal population of 50 cases who come to our hospital for physical examination were selected as control group. Serum procalcitonin combined blood gas analysis was performed in both groups,and the differences of serum procalcitonin and arterial blood gas were compared between the two groups. Results There was no significant difference between the control group and the observation group in the p H value and the oxygen saturation index( P〈0. 05). The other indexes showed significant changes( P〈0. 05). Compared with the control group,the observation group serum procalcitonin( 0. 099 ± 0. 041 ng/ml),carbon dioxide partial pressure( 77. 842 ± 25. 763 mm Hg) was significantly higher than the former,while the oxygen partial pressure( 51. 258 ± 21. 159 mm Hg) and the actual value of carbonate ions( 2. 537 ± 1. 126 mmol/L) was significantly lower than the former. Conclusion Serum procalcitonin combined with blood gas analysis has high value in predicting the clinical risk in patients with chronic obstructive pulmonary disease in acute exacerbation stage. The detection can help patients to diagnose chronic obstructive pulmonary disease,so it is worth popularizing in clinical application.
