摘 要:目的探讨治疗股骨颈骨折中髋关节置换术的应用与临床预后。方法纳入2011年2月—2016年6月40例股骨颈骨折患者以数字表法分组。对照组采用切开复位内固定手术治疗,观察组实施髋关节置换术治疗。比较两组股骨颈骨折治疗疗效;手术开展的时间、出血量、下床和术后住院的时间;干预前后患者髋关节功能HARRIS评分。结果观察组股骨颈骨折治疗疗效高于对照组,P<0.05;观察组手术开展的时间短于对照组,出血量少于对照组,下床和术后住院的时间短于对照组,P<0.05;干预前两组髋关节功能HARRIS评分相近,P>0.05;干预后观察组髋关节功能HARRIS评分优于对照组,P<0.05。结论治疗股骨颈骨折中髋关节置换术的应用与临床预后确切,可改善髋关节功能,减少创伤,缩短手术耗时和住院时间。Objective To discuss the application of hip arthroplasty in the treatment of femoral neck fracture and clinical prognosis. Methods 40cases of femoral neck fracture were grouped by digital table from February 2011 to June 2016. The control group was treated with open reduction and internal fixation, and the observation group was treated with hip arthroplasty. The curative effect of two groups of femoral neck fractures were compared. The time of operation, the amount of bleeding, the time of getting out of bed and postoperative hospitalization were compared between the two groups. The HARRIS score of hip joint function before and after intervention was also evaluated. Results The efcacy of treatment of femoral neck fracture in observation group was better than the control group, P 〈 0.05; in the observation group, the operation time was shorter than the control group, the bleeding was less than the control group, get out of bed and postoperative hospitalization time was shorter than the control group, P 〈 0.05; the HARRIS hip score of the two groups before the intervention had no significant statistics difference, P 〉 0.05; after the intervention, HARRIS hip score in the observation group was better than the control group, P 〈 0.05. Conclusion The application and clinical prognosis of hip arthroplasty in the treatment of femoral neck fractures is accurate, which can improve hip joint function, reduce trauma, shorten operative time and hospitalization time.
