基于脏腑别通之心胆同治治疗少阳头痛的临床观察

摘　　要：目的探讨小柴胡汤合酸枣仁汤为基本方随证加减并结合针刺足临泣穴、风市穴、神门穴治疗紧张型头痛中辨证为少阳头痛患者的疗效。方法采用随机对照方法,将符合纳入标准的紧张型头痛的患者分为治疗组和对照组。其中治疗组以小柴胡汤合酸枣仁汤为基本方随症加减;同时针刺足临泣穴、风市穴、神门穴。对照组根据2007年中国紧张型头痛诊疗专家共识予西医治疗方案。2周为一个疗程。分别于治疗前、治疗一周、二周进行数字分级法(NRS)疼痛评分、头痛病临床证候评价量表评分,最后对2组治疗前后的相关量表积分及中医证候疗效比较进行分析统计。结果治疗一周2组受试患者疼痛均明显缓解,伴随症状明显减少;治疗2周2组受试患者疼痛均完全缓解,伴随症状显著减少,治疗组临床症候评价较对照组显著改善。结论通过本研究验证这一治疗方案能较快缓解头痛,效果跟西药相当,但是缓解伴随症状,改善患者的体验明显优于西药组,不良反应极少,值得推广。Objective To investigate the curative effect of Xiaochaihu decoction suanzaoren decoction as basic prescription and acupuncture at Zulinqi（ G41）,Fengshi（ G31） and Shenmen（ TF4） in treating Shaoyang headache belongs to tension type. Methods A randomized control method was used. Patients with tension type headache met the inclusion criteria were divided into treatment group and control group. The treatment group was treated with Xiaochaihu decoction Suanzaoren decoction as basic prescription,at the same time,acupuncture at Zulinqi（ G41）,Fengshi（ G31） and Shenmen（ TF4）. The control group received Western medicine treatment according to the 2007 Chinese tension type headache diagnosis expert consensus. Two weeks was a course. Before treatment,treatment for one week and two weeks,Classification（ NRS） pain score,headache symptom assessment scale score,statistical analysis of relevant content on the final table of two groups before and after treatment and the curative effect of TCM syndrome integral were compared. Result After one week of treatment,pain of the two groups was significantly alleviated,symptoms decreased significantly. After two weeks of treatment,the pain of the two groups was completely relieved,accompanying symptoms significantly reduce the clinical symptoms. The evaluation of clinical symptoms in the treatment group was significantly improved than that in the control group. Conclusion This research verified that this treatment can quickly relieve the headache,the effect is comparable to that of western medicine,however,relieving the accompanying symptoms and improving the patients＇ experience are obviously better than those of the Western medicine group,and the adverse reactions are very few,so it is worth popularizing.
