血必净联合亚低温治疗对重型颅脑损伤患者预后及血清炎症因子水平的影响

摘　　要：目的观察血必净联合亚低温治疗对重型颅脑损伤患者血清中C反应蛋白(C-reactive protein,CRP)、白介素-6(interleukin receptors-6,IL-6)及肿瘤坏死因子-α(tumor necrosis factorɑ,TNF-ɑ)水平变化及患者预后的影响。方法选取新乡医学院第三附属医院2012年5月至2015年5月收治的重型颅脑损伤患者65例,随机分为对照组(32例)和观察组(血必净联合亚低温治疗,33例),观察两组患者治疗1、3、7d后血清CRP、IL-6和TNF-α水平的变化,并于治疗6个月后评价其预后。结果两组患者在治疗1d时,血清CRP、IL-6和TNF-α差异无统计学意义(P>0.05),观察组患者在3、7d时血清中CRP、IL-6和TNF-α水平明显低于对照组(P<0.05),观察组预后良好率(48.48%)与对照组(28.13%)比较也明显增高(P<0.05)。结论血必净结合亚低温治疗可明显降低重度颅脑损伤后血清CRP、IL-6和TNF-α水平,并可改善其预后。Objective To investigate the effect of Xuebijing combined with mild hypothermia on the prognosis and the expression of C-reactive protein,interleukin-6 and tumor necrosis factor alpha in patients with severe traumatic brain injury（TBI）. Methods Totally 65 patients with severe TBI were equally divided into two group： control group（n = 32）and observation group（Xuebijing combined with mild hypothermia,n = 33）. The serum levels of CRP,IL-6 and TNF-α were determined by enzyme linked immunosorbent assay in serum took in early morning from patients with TBI,and the prognosis was evaluated 6 months after treatment. Results The serum levels of CRP,IL-6 and TNF-α were no significant difference in patients in two groups（P〉0. 05） 1 day after treatment,but the serum levels of CRP,IL-6 and TNF-α were significantly lower in patients in obaervation group than those in control group（P〈0. 05） 3 days and 7 days after treatment. 6 months after treatment,the rate of good prognosis in the observation group was significantly higher than that in the control group（P〈0. 05）. Conclusion It is suggested that treatment of Xuebijing combined with mild hyperthermia can decrease the serum levels of CRP,IL-6 and TNF-α after severe TBI and improve the prognosis of the patients with severe TBI.
