CA199和总胆红素水平在胆管疾病应用中的临床分析

摘　　要：目的分析CA199和总胆红素水平在胆管癌与胆总管结石中的临床应用价值。方法回顾性分析郑州大学第一附属医院2016年2月至2017年2月于肝胆外科住院的75例胆管癌和79例胆总管结石患者的术前血清CA199及总胆红素水与相关性。结果 75例胆管癌中71例CA199阳性,79例胆总管结石中39例阳性,差异有统计学意义(χ2=38.686,P<0.05),两疾病间CA199水平和总胆红素水平分别相比较,差异有统计学意义(t=4.2、8.69,P<0.05),胆管癌中血清CA199与总胆红素无明显相关性(r=0.125,P=0.287),而胆总管结石中血清CA199水平与总胆红素相关性显著(r=0.512,P=0.00)。结论血清CA199联合总胆红素对胆管癌和胆管结石有一定的鉴别诊断意义,能够早期指导临床医师进行下一步诊治。Objective To explore the clinical value of CA199 and total bilirubin level in bile duct carcinoma and choledocholithiasis. Methods The serum CA199 and total bilirubin levels were analyzed statistically in 75 cases of cholangiocarcinoma and 79 cases of choledocholithiasis,the correlation of CA199 and total bilirubin in two diseases was analyzed in using correlation theory. Results The CA199 of 71 cases in 75 patients with bile duct carcinoma and 39 cases in 79 patients with bile duct calculus were positive. The difference was statistically significant（χ2= 38. 686,P〈0. 05）. Both CA199 levels and total bilirubin have obvious difference in patients in bile duct carcinoma and bile duct calculus（t =4. 2,8. 69,P〈0. 05）. The CA199 levels and total bilirubin have no obvious correlation in bile duct carcinoma patients（r = 0. 125,P = 0. 287）,but having obvious correlation in bile duct calculus（r = 0. 512,P = 0. 00）. Conclusion The CA199 levels combined with total bilirubin has certain diagnostic significance in bile duct carcinoma and bile duct calculus,it can guide early to the next step of diagnosis and treatmen for clinicians.
