左侧颈内静脉留置带涤纶套双腔导管22例体会  

Experience of 22 Cases Left Internal Jugular Vein Indwelling Double Lumen Catheter with Dacron Cuff

摘　　要：目的左侧颈内静脉留置带涤纶套双腔导管治疗分析。方法选自2010年2月至2017年2月本院因肾功能衰竭期维持性血透,因内瘘失功或不能行前臂内瘘建立术,且超声提示右侧颈内静脉狭窄或闭塞患者22例。结果 22例患者成功21例,其中1例因左颈内静脉狭窄置管手术失败,其余21例导管置入后均正常使用,无1例发生血胸、气胸、血气胸、空气栓塞、心率失常、出血等即刻并发症。结论本研究认为带涤纶套双腔管经左侧颈内静脉置管相对安全,使用寿命较长,血流量能够满足血液透析需要,对不能满足右侧颈内静脉置管条件的患者,是一种较好选择。Objective to analyze treatment of left internal jugular vein indwelling double lumen catheter with dacron cuff. Methods choose 22 cases right internal jugular vein stenosis or occlusion shown by ultrasound with renal failure period maintenance hemodialysis in our hospital from February 2010 to February 2017, who could not be treated with forearm fistula establishment for lacking internal fistula function. Results 21 cases were successful treated among 22 cases, 1 case failed with left internal jugular vein stenosis indwelling catheter, and catheter of other 21 cases work normally, no immediate complications of hemothorax, pneumothorax, hemothorax, air embolism, arrhythmia, bleeding etc. Conclusion The study suggests left internal jugular vein indwelling catheter with dacron cuff is relatively safe, with longer service life, and blood flow can meet needs of hemodialysis, which is a good choice for patients failing to meet conditions of right internal jugular vein indwelling catheter.
