摘　　要：我国慢性病发病人数快速上升,成为重大的公共卫生问题,慢性病防治工作仍面临着严峻挑战,慢性病防治网络尚不健全。广东省第二人民医院针对社区慢性病防控中存在的问题,通过建设社区健康小屋服务网点、开发远程健康监测网络、开展健康管理综合服务等措施,构建基于健康小屋的社区慢性病健康管理创新模式,对社区慢性病健康管理进行有益的探索。The number of chronic diseases in China has risen rapidly and has become a major public health problem.The prevention and control of chronic diseases is still facing severe challenges,and the network of prevention and control of chronic diseases is not yet sound.The problems exist in the prevention and control of chronic diseases in the community of the second People＇s Hospital of Guangdong Province,through the construction of community health home service outlets,the development of remote health monitoring network,and the development of comprehensive health management measures,a health management innovation model of chronic diseases in community based on healthy house is constructed,which will make a useful exploration for chronic disease health management in community.
