摘  要:目的调查急性加重入院慢性阻塞性肺疾病患者营养风险状况及与疾病预后的相关性。方法通过营养风险筛查评价工具——NRS200对362例急性加重入院慢阻肺疾病患者营养状态进行评估,入选患者按营养状况分成营养风险组和无营养风险组。分别观察两组间的体重指数(BMI)、血清白蛋白、肺功能、血气分析、肺部感染发生例数、抗感染治疗时间、合并呼吸衰竭例数、住院时间的差异。结果急性加重入院慢阻肺疾病患者的营养风险达65.19%。营养风险组患者的BMI、肺功能、血清白蛋白、血气分析、肺部感染发生例数、抗感染治疗时间、合并呼吸衰竭例数、住院时间与非营养不足组比有显著性差异(P<0.05)。结论急性加重入院慢阻肺疾病患者常常存在有营养风险状况,尤其是营养缺乏的高危人群,患者存在营养缺乏程度明显,肺功能越差,越易发生呼吸道感染、并发呼吸衰竭,导致住院时间延长,慢阻肺患者的营养状况与疾病的进展和预后相关。Objective To investigate the nutritional risk status of patients with acute exacerbation of chronic obstructive pulmonary disease( COPD) and its correlation with the prognosis of the disease. Methods The nutritional risk screening tool-NRS2002 was used to screen and assess the nutritional risk of 362 hospitalized patients with AECOPD. The patients were divided into the nutritional risk group and the non-nutritional risk group according to their nutritional status. Their body mass index( BMI),serum albumin,lung function,blood gas analysis,number of cases with pulmonary infection,anti-infection treatment time,number of cases with respiratory failure and the hospital stay were observed and compared between the two groups. Results The nutritional risk in patients with AECOPD was 65. 19%. BMI,pulmonary function,serum albumin,blood gas analysis and lung infection cases,anti-infection treatment time,combination of respiratory failure cases,and hospital stay in the nutritional risk group were significantly different with the non-nutritional risk group( P〈0. 05). Conclusion The hospitalized patients with AECOPD are high risk population who have nutritional risks,especially nutritional deficiencies. Patients with higher nutritional risk have worse lung function and longer hospital stay,and are more likely to develop respiratory infection and respiratory failure. The nutritional status of COPD patients affects the progression and prognosis of the disease.
