脂多糖结合蛋白联合CURB-65评分在社区获得性肺炎病情评估中的价值

摘　　要：目的研究脂多糖结合蛋白(LBP)联合CURB-65评分对社区获得性肺炎(CAP)病情评估价值。方法选取我院2014年1月到2017年1月间收治的CAP患者127例,根据其病情程度分为重症组(56例)和非重症组(71例),同时根据预后情况分为死亡组(20例)和存活组(107例),测定所有患者的血清LBP水平、CURB-65及APACHEⅡ评分,比较重症组和非重症组以及死亡组和非死亡组的血清LBP水平和CURB-65及APACHEⅡ评分,并分析其相关性。结果重症组的血清LBP水平、CURB-65及APACHEⅡ评分均明显高于非重症组(P<0.01),死亡组的血清LBP水平、CURB-65及APACHEⅡ评分均明显高于非死亡组(P<0.01);Pearson相关性分析显示,血清LBP水平和CURB评分均与APACHEⅡ评分呈明显正相关(r=3.181,4.723;P<0.01)。结论血清LBP水平、CURB-65评分与CAP患者病情呈明显正相关,能够一定程度提示死亡风险,对患者病情和治疗预后评估有着重要价值,值得在临床推广。Objective To study the effect of lipopolysaccharide binding protein（ LBP） combined with CURB-65 score in community-acquired pneumonia（ CAP） disease evaluation. Methods 127 CAP patients were selected in our hospital from January 2014 to January 2017. According to the severity of disease,they were divided into the severe group（ 56 cases） and the non severe group（ 71 cases）. And according to the prognosis,they were divided into the death group（ 20 cases） and the survival group（ 107 cases）. Their serum LBP levels,CURB-65 and APACHE Ⅱ score were determined in all patients,and the correlation was analyzed. Results The serum LBP levels,CURB-65 and APACHE Ⅱ were significantly higher in the severe group than in the non severe group（ P〈0. 01）,and higher in the death group than in the death group（ P〈0. 01）. Pearson correlation analysis showed that the serum LBP level and CURB scores and APACHE Ⅱ scores were positively correlated（ r = 3. 181,4. 723; P〈0. 01）. Conclusion The levels of serum LBP and CURB-65 score are positively correlated with the prognosis of patients with CAP,suggesting that the risk of death to a certain extent,which has important value to assess the prognosis of patients with the disease and treatment.
