强化饮食护理对维持性血液透析患者钙磷代谢的影响评价

摘　　要：目的评价强化饮食护理对维持性血液透析患者钙磷代谢的影响。方法选取2015-10/2016-12河南省新乡市第一人民医院行维持性血液透析治疗的62例血液透析患者,采用数字表法随机分为强化组和对照组,每组分别为31例,在原有临床治疗的基础上,强化组患者进行强化饮食护理及常规护理,对照组患者仅进行常规护理,3个月后对比两组患者自身护理前后的甲状旁腺激素、血清蛋白、血磷、血钙和钙磷乘积变化情况。结果两组患者护理前后血钙、血磷、甲状旁腺素、钙磷乘积差值相比,强化组均高于对照组,差异有统计学意义(P<0.05),而两组护理前后血钙变化差值相比,其差异无统计学意义(P>0.05)。强化组患者护理前后血清蛋白差值和血清总蛋白差值与对照组患者相比,差异无统计学意义(P>0.05)。结论对维持性血液透析患者开展强化饮食护理,能够改善积钙磷代谢情况,值得推广使用。Objective To evaluate the effect of intensive diet care on calcium and phosphorus metabolism in maintenance hemodialysis patients. Methods About 62 maintenance hemodialysis patients in our hospital from October 2015 to December 2016 were selected and randomly divided into intensive group and control group according to the digital table method, There were 31 cases in each group. On the basis of the original clinical treatment, intensive diet nursing and routine nursing were both carried out in the intensive group, and the patients in the control group were only given routine nursing care. After 3 months later, the parathyroid hormone, serum protein, blood phosphorus, blood calcium and calcium- phosphorus product of the patients in two groups before and after care were compared. Results The difference of blood calcium, blood phosphorus, parathyroid hormone and calcium - phosphorus product of the patients in the two groups, the intensive group was higher than the control group, and the difference was statistically significant （ P 〈 0. 05 ）. There was no significant difference in the difference of blood calcium changes between the two groups before and after nursing （ P 〉 0. 05 ）. Conclusion Intensive diet care for patients with maintenance hemodialysis can improve the metabolism of calcium and phosphorus, which is worth popularizing.
