百泌达对老年2型糖尿病病人的降糖效果研究

摘　　要：目的观察百泌达对老年2型糖尿病病人(T2DM)的降糖效果及安全性。方法选取我院150例初诊T2DM病人(初诊组)及135例长病程T2DM病人(长病程组),百泌达5μg,2次/d,给药12周,分别于8周及12周检测并比较2组的空腹血糖(fasting plasma glucose,FPG)、餐后2小时血糖(2 hour plasma glucose,2h PG)、空腹胰岛素(fasting insulin,Fins)、C肽、胰高血糖素、糖化血红蛋白(glycosylated hemoglobin,Hb A1c)、胰高血糖素样肽-1(glucagon-like peptide-1,GLP-1)的水平并计算胰岛素抵抗指数(HOMA-IR)及胰岛B细胞功能(HOMA-β)。结果 2组病人治疗8周后,与治疗前比较,血清GLP-1、Fins、C肽水平升高,胰高血糖素、FPG、2h PG、Hb A1c水平及HOMA-IR、HOMA-β稳态模型评估均明显降低(P均<0.05)。百泌达治疗12周后,血清的GLP-1、FPG、2h PG、Hb A1c、Fins、C肽水平及HOMA-IR与治疗8周相比差异无统计学意义。相关分析显示,血清Fins、C肽水平与GLP-1正性相关(r=0.73,0.66,P均<0.01),胰高血糖素水平与GLP-1负性相关(r=-0.73,P<0.01)。结论百泌达能有效控制老年T2DM病人的血糖,缓解胰岛素抵抗,恢复部分胰岛素功能。Objective To assess the efficacy of secretin in the treatment of type 2 diabetes mellitus（ T2 DM） in the elderly patients. Methods 150 patients with newly diagnosed T2 DM（ newly diagnosed group）,135 T2 DM patients with courses over five years（ long course group） were enrolled in this study. The two groups were gived secretin 5 μg,bid for 12 weeks. The levels of fasting plasma glucose（ FPG）,2 hour plasma glucose（ 2 h PG）, fasting insulin（ Fins）, glucagon, C-peptide,glycosylated hemoglobin（ Hb A1 c）,glucagon-like peptide-1（ GLP-1） were detected,and homeostasis model assessment-insulin resistance（ HOMA-IR） and were calculated and compared 8 and 12 weeks after the treatment in two groups. Results After 8 weeks treatment,the serum levels of GLP-1,Fins,HOMA-β and C-peptide were significantly increased and the levels of glucagon,FPG,2 h PG,Hb A1 c,HOMA-IR were significantly decreased in two groups（ P〈0. 05）. Correlation analysis showed that Fins HOMA-β and C-peptide were positively correlated with GLP-1（ r = 0. 73,r = 0. 66,all P〈0. 01）,and the glucagon was negatively correlated with GLP-1（ r =-0. 73,P〈0. 01）. Conclusions Secretin can effectively control blood glucose level in the elderly patients with T2 DM,improve insulin resistance,and recovery partial islet function.
