积极心理疏导对晚期宫颈癌化疗患者的负面情绪、生活质量及免疫状态的影响

摘　　要：目的：探讨积极心理疏导对晚期宫颈癌化疗患者的负面情绪、生活质量及免疫状态的影响。方法：收集2014年8月-2016年12月间在本院接受化疗的晚期宫颈癌患者78例,按照随机数表法分为对照组及观察组各39例。对照组患者接受常规干预,观察组患者在常规干预的基础上加入积极心理疏导。对比两组患者干预前后的负面情绪、生活质量、免疫状态差异。结果：干预前,两组患者的负面情绪、生活质量、免疫状态差异无统计学意义。干预后,观察组患者的焦虑自评量表（SAS）、抑郁自评量表（SDS）总分显著低于对照组患者（t=-8.802,-10.332;P〈0.01）;EORTC QLQ-C30各个功能领域评分值高于对照组患者（t=15.032,17.910,11.501,7.872,18.137;P〈0.01）;血清中Th1细胞因子白介素-2（IL-2）、干扰素-γ（INF-γ）、肿瘤坏死因子β（TNF-β）的含量高于对照组患者（t=7.691,5.325,7.719;P〈0.01）,Th2细胞因子白介素-4（IL-4）、白介素-6（IL-6）、白介素-10（IL-10）的含量低于对照组患者（t=-15.625,-14.622,-7.328;P〈0.05）。结论：积极心理疏导有助于缓解晚期宫颈癌化疗患者的负面情绪、提升生活质量、优化免疫状态。Objective：To study the effect of positive psychological counseling on negative emotions,quality of life and immune status in advanced cervical cancer patients underwent chemotherapy.Methods：A total of 78 cases of advanced cervical cancer patients underwent chemotherapy in our hospital during August 2014 to December 2016 were randomly divided into control group and observation group,each with 39 cases.Patients in control group were treated with routine intervention,while those in observation group were treated with positive psychological counseling combined with routine intervention.The negative emotions,quality of life and immune status were compared between two groups before and after intervention.Results：Before intervention,there were no significant differences in negative emotion,quality of life and immune status between two groups.After intervention,SAS,SDS scores in observation group were significantly lower than those in control group（t=-8.802,-10.332;P〈0.01）;EORTC QLQ-C30 functional areas scores in observation group were higher than those in control group（t=15.032,17.910,11.501,7.872,18.137;P〈0.01）;serum contents of Th1 cytokines such as IL-2,INF-γ,TNF-βwere higher than those in control group（t=7.691,5.325,7.719;P〈0.01）,content of Th2 cytokines such as IL-4,IL-6 and IL-10 were lower than those in control group（t=-15.625,-14.622,-7.328;P〈0.05）.Conclusion：Positive psychological counseling can relieve the negative emotion in advanced cervical cancer patients,improve the quality of life and optimize the immune status.
