摘  要:目的探讨腺苷脱氨酶(ADA)在1型糖尿病(T1DM)患者中与糖代谢、糖尿病并发症的关系。方法选择T1DM患者184例(T1DM组):其中根据合并并发症情况分为单纯1型糖尿病组(s T1DM组)、急性并发症组(aT1DM组)、慢性并发症组(cT1DM组)。同时纳入同期正常人群108例为对照组(CN组)。测定临床及生化指标,分析影响ADA水平的因素。结果 (1)T1DM组ADA水平高于CN组,cT1DM组ADA水平较aT1DM组增高(P<0.05)。(2)两两相关性分析:ADA与空腹血糖呈正相关,相关系数为0.156(P<0.05)。(3)Logistic回归分析显示,在T1DM患者中,ADA水平的升高是罹患慢性并发症的独立危险因素(OR:1.159,95%CI:1.044~1.286),是罹患急性并发症的保护性因素(OR:0.857,95%CI:0.748~0.981)。结论 T1DM患者的ADA水平较正常人群明显增高,同时ADA水平的增高是罹患糖尿病慢性并发症的独立危险因素,是罹患急性并发症的保护性因素。Objective To investigate the significance of serum adenosine deaminase level in type 1 diabe-tes patients(T1DM)with acute or chronic diabetes complications. Methods One hundred and eighty-four cases oftype 1 diabetes patients were divided into three groups:T1DM without any complications(n = 55),T1DM withacute diabetes complications(n = 47)and T1DM with chronic diabetes complications(n = 82). We also enrolled108 healthy persons as the control group. The clinical and biochemical data were collected and the influencing fac-tors of serum ADA were analyzed. Resultes(1)Serum ADA level of T1DM patients was higher than that of thehealthy control people. T1DM patients with acute diabetes complications had lower serum ADA level compared withT1DM patients with chronic complications.(2)Results of Pearson correlations analysis showed that serum ADAlevel had positive correlation with fasting blood glucose.(3)Logistic regression analysis showed that the increase ofserum ADA level was an independent risk factor in patients with chronic diabetes complications(OR:1.159,95%Cl:1.044~1.286),while it was a protective factor in patients with acute diabetes complications(OR:0.857,95%Cl:0.748~0.981). Conclusions High serum ADA level was observed in type 1 diabetes patients. The serumADA level was an independent risk factor for T1DM patients with chronic diabetes complications,while it was aprotective factor for T1DM patients with acute diabetes complications.
