短期应用米力农治疗难治性心力衰竭的临床效果观察

摘　　要：目的观察短期应用米力农治疗难治性心力衰竭的临床效果。方法选择难治性心力衰竭患者78例,随机分为对照组和观察组,每组39例。对照组予以常规抗心力衰竭药物治疗,观察组在常规治疗基础上加用米力农治疗,疗程均为7 d,比较2组治疗前后左室射血分数(LVEF)、每搏输出量(SV)、心脏指数(CI),N末端脑钠肽前体(NT-pro BNP)、超敏C反应蛋白(hs-CRP)、白介素6(IL-6)的水平及不良反应的发生情况。结果治疗7 d后,2组LVEF、SV、CI数值均高于治疗前,且观察组各项数值高于对照组(P均<0.01);2组NT-pro BNP、hs-CRP、IL-6水平均较治疗前均降低,且观察组降低更明显(P均<0.01)。2组不良反应发生率比较,差异无统计学意义(P>0.05)。结论短期应用米力农可以明显改善难治性心力衰竭患者的心功能,减轻炎性反应,且安全性好。Objective To observe the clinical effect of short-term use of milrinone in treatment of refractory heart failure. Methods 78 patients were randomly selected and divided into two groups,with 39 cases in each group. The control group was treated with conventional anti-heart failure drugs,and the observation group was treated with mirinone on the basis of routine treatment. The duration of treatment was 7 days. The left ventricular ejection fraction（ LVEF）,stroke volume（ SV）,cardiac index（ CI）,N-terminal pro-brain natriuretic peptide（ NT-pro BNP）,and high-sensitivity C-reactive protein（ hs-CRP） were compared between the two groups,interleukin 6（ IL-6） levels and the occurrence of adverse reactions were compared before and after treatment. Results After 7 days of treatment,the values of LVEF,SV and CI in the 2 groups were higher than those before treatment,and the values in the observation group were higher than those in the control group（ P〈0. 01）; 2 groups of NT-pro BNP,hs-CRP,IL-6 levels were lower than those values before treatment,and the observation group decreased more significantly（ P〈0. 01）. The incidence of adverse reactions in the two groups was not statistically significant（ P〉0. 05）. Conclusion The short-term use of milrinone can improve the heart function of patients with refractory heart failure,reduce inflammatory reaction,and has high security.
