摘  要:目的探讨老年2型糖尿病(type 2 diabetes mellitus,T2DM)患者血清甲基乙二醛(methyl ethyl two aldehyde,MG)和硫酸脱氢表雄酮(dehydroepiandrosterone sulfate,DHEA-S)水平与认知功能的相关性。方法选取2014年1月—2017年1月内蒙古医科大学附属医院收治的老年T2DM患者116例(观察组)和同期于我院体检正常老年人60例(对照组)作为研究对象,采用回顾性分析所有纳入研究者的临床资料,记录所有纳入研究者其性别、年龄、体质量指数等一般临床资料以及血清MG、DHEA-S水平和认知功能情况,并分析血清MG和DHEA-S与认知功能的相关性。结果观察组其DHEA-S水平和即时记忆、视觉广度、注意力以及延迟记忆评分均明显低于对照组,而观察组MG水平明显较对照组高(P<0.05);经Spearman等级相关分析进一步证实得MG和DHEA-S均与RBANS间存在着密切联系,其中MG与RBANS中的即时记忆、视觉广度、注意力以及延迟记忆评分均呈负相关,而DHEA-S与上述指标均呈正相关(P<0.05)。结论老年T2DM患者其MG和DHEA-S水平均分别显著升高和下降,且MG和DHEA-S与老年T2DM患者的认知功能均分别呈负相关和正相关存在,故该2项指标可作为临床上评估老年T2DM患者认知功能的有效指标。Objective To investigate the cor relations between serum levels of methylglyoxal(MGO) and dehydroepiandrosterone sulfate(DHEA-S) and cognitive function in elderly patients with type 2 diabetes mellitus(T2DM). Methods One hundred and sixteen cases of elderly T2DM patients(observation group) and another 60 normal elderly patients(control group) who were in the Affiliated Hospital of Inner Mongolia Medical University in the same period between January 2014 to January 2017 were selected as the subjects. The clinical data, such as sex, age, body mass index, serum MGO, DHEA-S levels and cognitive function, were recorded before the correlations between serum MGO and DHEA-S and cognitive function were analyzed. Results DHEA-S levels and scores of immediate memory, breadth of vision, attention span and delayed memory in the observation group were significantly lower or poorer than those of the normal group, while the level of MGO in the observation group with T2DM was significantly higher(P〈0.05). The Spearman rank correlation analysis confirmed that MGO and DHEA-S were closely related to RBANS. MGO and RBANS were negatively related to scores of immediate memory, visual range, attention span, and delayed memory, while DHEAS was positively correlated with the above indicators(P〈0.05). Conclusion The levels of MGO and DHEA-S in elderly patients with T2DM are significantly increased and decreased respectively. MGO and DHEA-S are negatively and positively correlated with cognitive function in elderly patients with T2DM respectively, so the two indicators can be used to assess the cognitive function of elderly patients with T2DM.
