319例危重型手足口病患儿硝普钠药学监护回顾性分析

摘　　要：目的探讨硝普钠治疗危重型手足口病的效果与合理使用方法。方法对医院2015年1月至12月住院使用硝普钠的319例危重型手足口病患儿的药学监护资料进行回顾性调查分析,调查内容包括硝普钠的初始剂量、用药时间、疗程、不良反应等。结果 319例使用硝普钠的患儿中,给药时间2~7 d,平均4.76 d;给药剂量1~10μg/(kg·min),平均6.78μg/(kg·min);呼吸机辅助治疗7例(2.19%),死亡1例(0.31%);治疗有效318例(99.69%),无效1例(0.31%);出现药物常见不良反应64例(20.06%),其中烦躁62例(96.88%),低血压、耐药各1例(1.56%)。结论硝普钠治疗危重型手足口病疗效确切,常见不良反应发生率高,儿童使用时需加强药学监护。Objective To investigate the clinical effect of sodium nitroprusside in the treatment of children with severe hand-foot-and-mouth disease and its rational use. Methods Totally 319 children with severe hand-foot-and-mouth disease admitted to our hospital from January to December 2015 were treated with sodium nitroprusside,and the pharmaceutical care data of 319 children were analyzed retrospectively. The investigation included initial dose of sodium nitroprusside,duration of medication,course of treatment and adverse reactions. Results For 319 children patients treated with sodium nitroprusside,the time of administration was 2-7 d,with an average of 4. 76 d,the dosage was 1-10 μg/（ kg·min）,with an average of 6. 78 μg/（ kg·min）. 7 cases（2. 19%） were treated with ventilator,1 case（ 0. 31%） died,318 cases were effective（ 99. 69%） and 1 case（ 0. 31%） was invalid. 64 cases（ 20. 06%） had common adverse drug reactions,of which 62 cases were irritable（ 96. 88%）,1 case（ 1. 56%） was hypotension and 1 case（ 1. 56%） was drug resistance.Conclusion Sodium nitroprusside is effective in the treatment of severe hand-foot-and-mouth disease,and the incidence rate of common adverse reactions is high. Pharmaceutical care should be strengthened when sodium nitroprusside is used for treating children.
