摘  要:目的探讨声辐射力脉冲成像(ARFI)技术观察低温后复温兔肾脏改变的应用价值。方法将16只新西兰大白兔置于-25℃环境中8 h,然后置于23℃环境中复温,分别于常温和低温8 h(T_0)以及低温后1 h、2 h、3 h、4 h、5 h、6 h、7 h、8 h(T_1、T_2、T_3、T_4、T_5、T_6、T_7、T_8)共9个时间点对实验兔进行右肾常规二维超声检查,观察肾实质回声,测量肾脏大小,并计算肾脏体积,同时采用ARFI技术测量肾实质SWV。采用独立样本t检验分别比较不同时间点实验兔肾脏体积、肾实质SWV。结果各时间点实验兔肾皮质回声均未发生明显改变。T_1~T_8各时间点与T_0时间点实验兔肾脏体积差异均无统计学意义。T_1~T_8各时间点实验兔肾实质SWV均小于T_0时间点,且差异均有统计学意义(t=-8.455、-10.608、-6.664、-6.146、-10.483、-8.005、-6.021、-7.942,P均<0.001)。T_1时间点实验兔肾实质SWV较T_0时间点急剧下降,且差异有统计学意义(t=-8.455,P<0.001)。T_2时间点实验兔肾实质SWV大于T_1时间点,T_3时间点实验兔肾实质SWV大于T_2时间点,且差异均有统计学意义(t=2.496,P=0.025;t=3.362,P=0.004);T_3~T_8时间点实验兔肾实质SWV逐渐减小,且任意相邻时间点间SWV差异均无统计学意义。这说明,低温后复温,随着时间变化,实验兔肾实质SWV先减小,后升高,然后再降低。结论 ARFI技术可反映低温后复温实验兔肾脏的改变,可为临床提供更多有用的信息。Objective To explore the value of acoustic radiation force impulse imaging(ARFI)technique in the evaluation of the change of kidney in rabbits with hypothermia and rewarming.Methods Sixteen New Zealand rabbits were placed in-25℃environment for 8 hours,then rewarmed in23℃environment.Ultrasonic examinations were performed at room temperature in the time of T_0 and the time of T_1-T_8.The right kidney were evaluated by two-dimensional ultrasound and ARFI technique.Results No significant changes were found in the echo and volume of kidney from T_1 to T_8.SWV of kidney descended greatly from T_0 to T_1 and raised from T_2 to T_3 gradually,then it descended from T_3 to T_8 gradually.Significant difference of SWV of kidney were found from T_1 to T_8 compared with that at T_0(P〈0.05).Conclusions ARFI Technique can estimate the change of kidney in rabbits with hypothermia and rewarming.It can provide more useful information for clinical diagnosis.
