经尿道160W直光束绿激光前列腺逆行剥离汽化术治疗良性前列腺增生症的临床研究

摘　　要：目的探讨改良经尿道160W直光束绿激光逆行剥离汽化术在良性前列腺增生症(benign prostatic hyperplasia,BPH)所致膀胱出口梗阻治疗中的疗效及安全性。方法回顾性研究2014年10月至2015年10月期间本中心采用经尿道160 W直光束绿激光逆行剥离汽化术治疗的86例BPH患者,记录患者围术期数据及术后1个月、3个月、6个月及12个月的随访情况,分析数据并进行统计学处理。结果手术均顺利完成,平均手术时间为(52.4±15.0)分钟。患者术后国际前列腺症状评分、最大尿流率及残余尿较术前数据均显著改善(P<0.01)。导尿管留置时间和住院时间均较短,手术前后无严重并发症发生,所有患者均无输血。术后随访方面,出现前列腺包膜穿孔3例,膀胱颈挛缩2例,尿道狭窄2例,无其余严重并发症。结论改良经尿道160W直光束绿激光逆行剥离汽化术治疗BPH所致的膀胱出口梗阻,是安全和有效的。Objective To evaluate the technical improvement and experience of the 160W straight beam LBOlaser photoselective retrograde stripping - vaporization of the prostate （ PRSVP）for the surgical treatment of benign prostatic hyperplasia （ BPH） . Methods From October 2014 to October 2015,a total of 86 patients undergoing PRSVP were included in this retrospective study. All patients were preoperatively assessed by related data.Perioperative parameters and complications were recorded. Patients were reassessed at 1,3,6 and 12 monthspostoperatively. Results PRSVP resulted in a significant improvement of International Prostate Symptom Score（ IPSS） ,maximum flow rate （Qmax） ,and post - void residual urine （ PVR） . Mean operative time was（ 52. 4 ± 15. 0） minutes for PRSVP. A significant improvement in favor of PRSVP was achieved regarding thecatheter indwelling time and hospital stay time. No severe perioperative complications occurred. There is no requiring blood transfusion in all patients. Capsule perforation was observed in 3 patients in the group. There were2 patients in the group experienced bladder neck contracture and 2 patients were diagnosed urethral stricture.Conclusions 160W straight beam LBO laser PRSVP is effective in treatment option for symptomatic BPH. PRSVP is favourable in terms of perioperative safety and results in short postoperative catheter time and hospital stay.
