摘  要:目的:研究骨折内固定术后髓内感染患者行万古霉素骨水泥治疗临床效果。方法:选取2015年6月~2017年11月期间洛阳市中心医院收治的骨折内固定术后髓内感染患者98例作为研究对象,按照随机数字表法将其分为实验组(n=49)、对照组(n=49),给予实验组患者万古霉素骨水泥治疗,给予对照组患者抗生素持续灌注治疗,观察两组患者的临床治疗效果、复发率、疼痛情况评分、生活质量、创口愈合情况、感染控制时间、住院时间、相关指标(白细胞计数、C蛋白反应、血沉)。结果:临床治疗效果对比显示实验组高于对照组,P<0.05。复发率对比显示实验组低于对照组,P<0.05。疼痛情况评分对比显示实验组低于对照组,P<0.05。生活质量对比显示实验组高于对照组,P<0.05。创口愈合时间对比显示实验组低于对照组,P<0.05。感染控制时间对比显示实验组低于对照组,P<0.05。住院时间对比显示实验组低于对照组,P<0.05。相关指标对比显示实验组低于对照组,P<0.05。结论:骨折内固定术后髓内感染患者行万古霉素骨水泥治疗临床效果显著,创面愈合较好,值得临床推广和应用。Objective: To study the clinical effect of vancomycin bone cement in patients with intramedullary infection after internal fixation of fracture. Methods: 98 patients with intramedullary infection after internal fixation from June 2015 to November 2017 were studied. According to the method of random number table, it was divided into experimental group(n=49) and control group(n=49). The patients in the experimental group were treated with vancomycin bone cement, and the patients in the control group were treated with antibiotic continuous infusion. The clinical therapeutic effect and recurrence rate of the two groups were observed. Pain score, quality of life, wound healing, infection control time, length of stay, correlation index(leukocyte count C protein reaction, erythrocyte sedimentation rate) were studied. Results: The results showed that the clinical effect in the experimental group was better than the control group(P〈0.05). The recurrence rate in the experimental group was lower than that in the control group(P〈0.05). The pain score in the experimental group was lower than that in the control group(P〈0.05).The quality of life in the experimental group was higher than that in the control group(P〈0.05). The wound healing time in the experimental group was lower than that in the control group(P〈0.05). The control time of infection in the experimental group was lower than that in the control group(P〈0.05).The length of hospitalization in the experimental group was lower than that in the control group(P〈0.05). The relative indexes were lower in the experimental group than in the control group(P〈0.05). Conclusion: The treatment of intramedullary infection with vancomycin bone cement after internal fixation of fracture is effective, and the wound healing is better, so it is worth popularizing and applying.
