摘 要:目的探讨系统性红斑狼疮(SLE)患者外周血IL-17^+αβT细胞、IL-17^+γδT细胞的表达及其与疾病活动性的关系。方法收集30例SLE患者和19例健康人(对照组)的临床资料,用流式细胞仪检测外周血αβT细胞、γδT细胞及IL-17^+的外周血单个核细胞(PBMC)、IL-17^+αβT细胞、IL-17^+γδT细胞的百分率,探讨它们与SLE疾病活动性的关系。结果 SLE患者IL-17^+αβT细胞和IL-17^+γδT细胞分别占IL-17^+PBMC的(71±21)%和(12±11)%。对照组IL-17^+αβT细胞和IL-17^+γδT细胞分别占IL-17^+PBMC的(55±27)%和(11±11)%。SLE患者IL-17^+PBMC及IL-17^+αβT细胞百分率较对照组均显著升高(P<0.01),SLE患者外周血中γδT细胞百分率较对照组显著下降(P<0.01)。SLE患者的αβT细胞及IL-17^+PBMC、IL-17^+αβT细胞、IL-17^+γδT细胞百分率与疾病活动性呈正相关(r_(αβT)=0.46,P_(αβT)<0.01;r_(IL-17 PBMC)^+=0.47,P_(IL-17 PBMC)^+<0.01;r_(IL-17αβT)^+=0.53,P_(IL-17αβT)^+<0.01;r_(IL-17γδT)^+=0.42,P_(IL-17γδT)^+<0.05),γδT细胞百分率与初发狼疮相关(r=0.43,P<0.05),但与疾病活动性无关(r=-0.67,P>0.05)。结论人PBMC中IL-17主要由αβT细胞产生,αβT细胞、IL-17^+PBMC、IL-17^+αβT细胞、IL-17^+γδT细胞百分率可反映狼疮病情活动。Objective To investigate the expression of IL - 17 - producing αβ T cells and IL - 17 - producing γδ T cells in the peripheral blood of systemic lupus erythematosus (SLE) patients and their correlations with disease activity. Methods The clinical data were collected and the percentages of αβ T cells, γδ T cells, IL - 17 + peripheral blood mononuclear ceils(PBMC) , IL- 17 + αβ T ceils and IL- 17 + γδ T ceils were detected in the peripheral blood of 30 SLE patients and 19 healthy controls by flow cytometry. And their.correlations with disease activity were analyzed. Results In SLE patients, the percentages of IL - 17 αβ T cells and IL - 17 +γδ T cells in IL - 17 + PBMC were ( 71 ± 21 ) % and ( 12 ± 11 ) % , respectively; while in the control group, the percentages of IL - 17 + αβT cells and IL - 17 +γδ T cells in IL- 17 + PBMC were lower ((55 ±27) % and (11 ± 11 ) %, respectively). Compared to the control group, both the percentages of IL - 17 + PBMC and IL - 17 +αβ T cells in the SLE group increased significantly ( P 〈 0.01 ). But the per- centage of γδ T cells in the SLE group was statistically significantly lower ( P 〈 0.01 ). As a diagnostic indicator for active SLE ( SLEDAI ≥ 10), the percentages of αβT cells, IL - 17 + PBMC, IL - 17 +γδT cells and IL - 17 +γδ T cells were correlated with disease activity ( rαβ T = 0.46, Pαβ 〈 0. 01 ; rIL-17 +PBMC = 0. 47, PIL-17 +PBMC 〈 0. 01 ; rIL-17 +αβT = 0. 53, PIL- 17 + αβ T 〈 0. 01 ; rIL- 17 + αβ T = 0. 42, PIL 17 + γδ T 〈 0. 05, respectively). The percentage of γδ T cells was correlated with initial diagnosis positively ( r = 0. 43, P 〈 0. 05 ), but not significantly correlated with disease activity ( r = - 0. 67, P 〉 0. 05). Conclusion IL - 17 in human PBMC is mainly produced by αβ T cells. The percentages of αβ T cells, IL - 17 PBMC, IL - 17 +αβ T cells and IL - 17 + γδ T cells in the peripheral blood
