摘　　要：目的评估消除隔阂计划对社区精神分裂症病耻感、生存质量以及家庭负担的影响。方法系统抽样方法抽取90列病例,按1∶1配对,干预组和对照组各45例。在入组前、干预第6、12和18个月后,采用贬低-歧视感知量表(PDD)、WHO生存质量简明量表(WHOQOL-BREF)和家庭疾病负担量表(FBS)分别评估患者及家属病耻感情况、患者的生存质量以及家属的家庭负担情况。结果第6个月、12个月、18个月时,干预组患者及其家属的PDD评分低于对照组(P<0.05或P<0.001);干预组患者的生理健康、心理健康和社会关系评分高于对照组(P<0.05或P<0.001),周围环境2组评分差异无统计学意义(P>0.05),干预组家属FBS评分低于对照组(P<0.05或P<0.001)。结论实施消除隔阂计划能显著改善患者及家属的病耻感,提高患者的生存质量,并减轻家属的家庭负担。Objective To evaluate the effect of the elimination of barriers initiative on stigma of community patients with schizophrenia and their families,quality of life of patients and their family burdens. Methods 90 patients selected through systematic sampling were equally divided into the intervention group and the 1 ∶ 1 matched control group. The patients were investigated by the perceived devaluation-discrimination scale（PDD）,the WHO quality of Life-BREF scale（WHOQOL-BREF） and family burden scale of disease（FBS） after 6,12 and 18 months＇ intervention to evaluate the patients＇ and families＇ stigma,patients＇ quality of life and family burdens between two groups. Results After 6,12 and 18 months＇ intervention,the patients＇ and families＇ PPD scores in the intervention group were both lower than those in control group（P〈0. 05 or P〈0.001）. Physical health,mental health and social relationship scores of the intervention group were higher than those in control group（P〈0. 05 or P〈0. 001）,but there is no significant difference between two groups in around environment scores（P〈0.05）. The FBS score in the intervention group was lower than that in control group（P〈0. 05 or P〈0. 001）. Conclusion Elimination of barriers initiative intervention can improve patients＇ and families＇ stigma and patients＇ quality of life and decrease family burdens.
