摘  要:目的探讨甘油化红细胞冻存前保留或去除细胞外多余甘油溶液对解冻后红细胞质量影响的差异。方法采取配对设计的方法,以甘油化红细胞冻存前保留细胞外多余甘油溶液为试验组,去除细胞外多余甘油溶液为对照组,制备生成6对红细胞添加液(甘露醇-腺嘌呤-磷酸盐,简称MAP)悬浮和6对0.9%Na Cl悬浮的冰冻解冻去甘油红细胞,比较分析其新鲜制备后甘油残留量、血红蛋白含量和游离血红蛋白(FHb)含量的差异,以及保存期间FHb含量和上清钾离子(K+)浓度的差异。结果新鲜制备的冰冻解冻去甘油红细胞,甘油残留量、FHb含量和血红蛋白含量均符合国家质量标准要求,试验组与对照组之间甘油残留量,FHb含量差异均无统计学意义,受实验过程留样损失的影响,试验组与对照组之间Hb含量差异有统计学意义;保存期间MAP悬浮和0.9%Na Cl悬浮冰冻解冻去甘油红细胞,上清K+浓度和FHb含量均呈现试验组低于对照组的总体趋势,其中MAP悬浮冰冻解冻去甘油红细胞,制备后21 d、28 d,试验组与对照组FHb含量差异具有统计学意义(P〈0.05),制备后7 d上清K+浓度差异具有统计学意义(P〈0.05),0.9%Na Cl悬浮冰冻解冻去甘油红细胞,制备后12 h、24 h试验组与对照组FHb含量差异具有统计学意义(P〈0.05),制备后6 h、12 h、24 h上清K+浓度差异具有统计学意义(P〈0.05)。结论甘油化红细胞冻存前保留或去除细胞外多余甘油溶液对新鲜制备冰冻解冻去甘油红细胞质量的影响没有显著差异,但随着保存时间的延长前者表现出更有利于红细胞稳定性保持的特点。Objective To investigate the effect of redundant glycerol removal before storage on the quality of thawed deglycerolized RBCs. Methods Deglycerolized RBCs with redundant glycerol removed before storage were set up as the control group while deglycerolized RBCs stored with glycerol functioned as the control group. We prepared 12 pairs( 1 experiment sample and 1 control sample each pair) of deglycerolized RBC sets,which were randomly re-suspended in 0. 9% Na Cl( n= 6) and MAP( n= 6). Glycerol residues,hemoglobin content,and FHb content in supernatant between two groups were compared upon preparation of the deglycerolized RBC units. During storage( 2—6℃),the RBCs were analyzed regarding the concentration of K+and FHb contents in supernatant for stability evaluations. Results Glycerol residue,FHb contents and hemoglobin contents of all the units met the national quality standard requirements. There were few statistically significant differences between the two groups in quality evaluations except the hemoglobin contents. The hemoglobin contents in the two groups were significantly different due to retention samples loss. During cold storage,the concentration of K+and FHb content revealed an overall trend where experimental group presented consistent lower values than that of the control group. To be more specific,deglycerolized RBC pairs in MAP presented significant difference at 21 d and 28 d after preparation( P 0. 05) in FHb contents and at d7 post preparation in the concentration of K+( P〈0. 05). As for the 0. 9% Na Cl suspended RBC pairs,FHb contents showed significant differences at 12 h and 24 h post preparation( P〈0. 05) and the concentrations of K+ were significantly different at 6 h,12 h and 24 h post preparation( P〈0. 05). Conclusion The quality of freshly prepared thawed deglycerolized RBCs showed little influence by the removal of supernatant glycerol before storage. However,with a longer preservation time,the removal operation favored improved RBC stabil
