摘　　要：目的了解山东省浴池来源的男男性行为者（MSM）的特征、艾滋病病毒（HIV）感染及行为变化,探索HIV感染的影响因素。方法选择2011-2015年全省4个市浴池来源MSM,分析其人口学特征、行为变化及HIV感染的影响因素。采用χ_（趋势）~2检验进行人口学特征、行为学指标等的趋势分析,应用多因素Logistic回归进行HIV感染因素的影响因素分析。结果2011-2015年分别调查402、563、336、201、238名MSM,HIV感染率分别为5.0%、3.0%、4.5%、9.0%和5.5%,差异无统计学意义（χ_（趋势）~2=2.860,P=0.091）;梅毒感染率分别为14.2%、5.9%、8.0%、7.5%、5.9%,呈下降趋势（χ_（趋势）~2=9.060,P=0.003）。最近6个月与同性肛交行为比例从2011年的86.9%上升至2015年的96.6%（χ_（趋势）~2=23.120,P〈0.001）;最近6个月肛交时未坚持使用安全套的比例从2011年的72.8%下降至2015年的65.2%（χ_（趋势）~2=6.910,P=0.009）;最近6个月有同性商业性行为的比例由2011年的38.3%下降至2015年的11.3%（χ_（趋势）~2=48.205,P〈0.001）;吸毒比例由2011年的2.0%上升至2015年的7.6%（χ_（趋势）~2=12.540,P〈0.001）。单因素分析显示,年龄〉30岁、离异或丧偶、本地居住时间〈3个月、最近6个月发生同性商业性行为时未坚持使用安全套、感染梅毒,是MSM感染HIV的重要影响因素;多因素Logistic回归分析显示,年龄〉30岁、离异或丧偶、本地居住时间〈3个月、感染梅毒,是HIV感染的危险因素。结论浴池MSM的高危行为比例仍相对较高,需要高度关注浴池内30岁以上、离异或丧偶、既往感染过梅毒的流动MSM。Objective To understand the characteristics,behaviors,and HIV infection among men who have sex with men（MSM）haunting bathhouses during 2011-2015 in Shandong province,in order to provide evidence for AIDS control among the group.Methods Four cities of Shangdong province was selected to recruit the MSM as the research objects,so as to analyze their demographic characteristics,behavioral changes and HIV infection.Trend analysis was made by Chi-square test to know the demographic characteristics,behavioral indicators,and multivariate logistic regression analysis was conducted for factors affecting HIV infection.Results During 2011-2015,a total of 402,563,336,201 and 238 MSM were investigated,with HIV prevalence rates of 5.0%,3.0%,4.5%,9.0% and 5.5%（χ~2=2.860,P=0.091）,and syphilis infection rates showed a downward trend（11.3%,4.6%,6.9%,6.9%,and 5.3%）（χ~2=9.060,P=0.003）.In the last 6 months,the proportion of homosexual anal intercourse increased from 86.9%in 2011 to 96.6% in 2015（χ~2=23.120,P0.001）;the proportion of inconsistent condom use in the last 6 months dropped from 72.8% in 2011 to 65.2% in 2015（χ~2=6.910,P=0.009）;The proportion of commercial homosexualintercourse in the last 6 months decreased from 38.3% in 2011 to 11.3% in 2015（χ~2=48.205,P0.001）;the proportion of drug abuse increased from 2.0% in 2011 to 7.6% in 2015（χ~2=12.540,P0.001）.Univariate analysis showed that the important factors affecting HIV infection included inconsistent condom use in homosexual intercourse in the past 6 months,age 30 years,being divorced or widowed,local residence time 3 months,and syphilis infection.Logistic regression analysis showed that the risk factors for HIV infection were age 30 years old,being divorced or widowed,local residence time3 months,syphilis infection.Conclusion The proportion of high-risk behaviors among MSM haunting bathhouses is relatively high and it is necessary to pay close attention to the MSM group esp.those aged over 30 years,divorced or widowed and previously inf
