摘 要:目的分析乳腺钼靶表现为非肿块病变的MRI影像学特点,判断病变良、恶性,探讨MRI对乳腺非肿块病变的诊断价值。方法收集行乳腺钼靶检查诊断为非肿块性病变且术前行乳腺MRI检查的患者58例,观察分析其MRI中的病灶形态、弥散加权及增强特点、时间-曲线等。结果58例乳腺钼靶表现为非肿块病变的患者中,BI—RADS4-5类13例,其中7例病理证实为恶性病变,均有伴随征象,如钙化(集群分布钙化、段样分布钙化)、局部的结构扭曲、相邻皮肤增厚以及乳头内陷,6例单纯非肿决病变中3例为良性病变;BI—RADS2-3类25例,其中10例病理证实为恶性病变;BI—RADS0类20例,需要进一步行其他影像学检查;病理符合率为43.10%。58例行MRI结果显示,BI—RADS4-5类31例,25例病理证实为恶性病变;BI—RADS2-3类27例,病理证实3例为恶性病变;MRI病理符合率为84.95%。结论对乳腺钼靶表现为非肿块性的病变,乳腺MRI通过对其病变形态、弥散加权与动态增强、时间-信号强度进行综合分析,对判断病变的良、恶性具有重要意义。Objective To analyze the MRI imaging features of non-mass lesions shown by mam-mographic, and to judge the benign and malignant of the lesions and discuss the value of MRI in the diag- nosis of non-mass lesions of the breast. Methods Fifty-eight patients who were diagnosed as non-mass lesions by mammographic and underwent breast MRI examination before operation were enrolled. The le- sion morphology, diffusion weighted and enhancement features, time curve and other in MRI were ob- served and analyzed. Results Among the 58 cases of mammography with non-mass lesions, 13 cases were BI-RADS category 4 and 5, and 7 cases were confirmed by pathology. All of the 7 cases were ac- companied with signs, such as calcification (clustered calcification, segmental calcification), partial structural distortion, thickening of adjacent skin and nipple invasion, and 3 cases in 6 cases of simple non-mass lesions were benign lesions. In 25 cases of class BI-RADS category 2 and 3, 10 cases were ma- lignant lesion by pathology; 20 cases of BI-RADS 0 need further imaging examination; the coincidence rate was 43.10%. Fifty-eight cases of MRI results showed that, 31 cases were BI-RADS category 4 and 5, and 6 cases of them were benign lesion by pathology ; 27 cases were BI-RADS category 2 and 3, and 3 cases of them were malignant lesion by pathology; the pathological coincidence rate of MRI was 84.95%. Conclusions Mammography is a non mass lesion. Mammography MRI is of great significance in judging the benign and malignant lesions by analyzing the lesion shape, diffusion-weighted and dynam- ic enhancement and time signal intensity.
