摘 要:目的:探求比例法检测结核杆菌药敏试验中的最适菌液浓度。方法:通过收集2014年~2017年期间107株结核分枝杆菌菌株,并以GeneXpert检测法对RIF敏感性的检测结果作为金标准,利用一系列的倍比稀释方法,用无菌生理盐水分别对各菌株制备出四组共8种菌液浓度,对比不同组别菌液在中性罗氏对照培养基的菌落生长情况,以及各组别菌液浓度下比例法药敏试验的敏感度、特异度、准确率。结果:原对照组、1号实验组、2号实验组、3号实验组这四个组别在中性罗氏对照培养基菌落上的生长情况,合格率分别为86.0%、100%、100%、100%:而以上各稀释浓度下检测RIF药敏试验的正确率分别为83.2%、99.1%、88.8%、83.2%。结论:比例法检测结核杆菌药敏试验中的最适实验菌液浓度是2×10^-2g/L、2×10^-4/L。各实验室应该针对自身的实际情况或经验,探求适宜的试验菌液浓度,以此对比例法进行适当的改良。Objective: To determine the optimum concentration of bacterial liquid in the drug sensitivity test of Mycobacterium tuberculosis. Methods: 107 strains of Mycobacterium tuberculosis (MTB) strains were collected from 2014 to 2017 in this study. Rifampicin (RIF) sensitivity by GeneXpert detection were used as the gold standard.8 kinds of MTB liquids with different concentration by doubling dilution were inoculated in the neutral Roche medium. The growth condition was used to evaluate the sensitivity, specificity and accuracy. Results: The qualified rates of the control group, No.1, No.2 and No.3 experimental groups in the neutral Roche medium were 86.0%, 100%, 100% and 100% respectively; The accuracy rates in the drug sensitivity tests of RIF were 832%, 99.1%, 88.8%, 83.2%, respectively. Conclusion: The optimum experimental bacteria concentrations in the antimicrobial sensitivity test of MTB to RIF were 2~ 102 g/L and 2~I0-~ g/L. Laboratories should evaluate the optimum concentrations of MTB to improve the test performance based on their own circumstances and practice.
