阴道上皮内瘤变与TCT、高危型HPV感染、宫颈病变的关系探讨

摘　　要：探讨阴道上皮内瘤变(VaIN)与TCT、高危型HPV感染、宫颈病变的关系。收集2015年1月~2017年1月因细胞学筛查异常行阴道镜和组织学检查,诊断为VaIN的患者128例。计数资料组间比较应用卡方检验和Kruskal Wallis检验。(1)VaIN患者中≥40岁者高度病变发现率与<40岁者高度病变发现率差异有显著性;(2)VaIN患者中,单一感染与多重感染差异有显著性;(3)VaIN的高度病变在TCT为LSIL及以下者中的发现率与在TCT为ASC-H、HSIL及癌者(以下记做≥ASC-H)的发现率差异有显著性;(4)宫颈病理为低度病变及以下者VaIN的高度病变发生率与病理为高度病变及以上者,VaIN的高度病变发生率差异有显著性。VaIN与高危型HPV感染及宫颈病变的关系密切,在临床中应重视对阴道上皮的检查。To discuss the relationship of vaginal intraepithelial neoplasia（VaIN）,thinprep cytologic test（TCT）,high-risk human papilloma virus（HR-HPV）and cervical intraepithelial neoplasia（CIN）.A total of 128 cases of VaIN patients were retrospectively analyzed with abnormal examination results from Jan,2015 to Jan,2017.They were all proceeded colposcopy and histological examination.Theχ2-test and Kruskal Wallis test were used.（1）The VaIN of HSIL in 40 or more than 40 years old patients was significantly higher than in less than 40 years old patients;（2）Among 128 patients,there was a significant difference between the single infection rate and the multiple one;（3）The HSIL of VaIN in high class or cancer of TCT was significantly higher than in low class;（4）The HSIL of VaIN in high class patients of pathological diagnosis was significantly higher than in low class patients.VaIN,HR-HPV,and CIN were closely and we should more attention to the examination of vaginal epithelium in clinical work.
