摘 要:目的:探讨不同胎龄新生儿出生后0~6月骨生化标志物、胰岛素样生长因子-I(IGF-I)的变化趋势及与体格发育指标的相关性。方法:109例新生儿根据胎龄分为早期早产儿组(37例)、晚期早产儿组(41例)及足月儿组(31例),于生后1 h、7 d、3月龄及6月龄时检测3组新生儿的血清维生素D3(VitD_3)、骨源性碱性磷酸酶(BAP)、钙、磷及IGF-I,同时测量体格指标(身长、头围、体质量),分析血清VitD_3、BAP、钙、磷及IGF-I与体格发育的关系。结果:随日龄增加,3组新生儿的血清VitD_3、钙水平增高,早产儿组血清IGF-I水平增高(P<0.05);随胎龄、日龄增加,3组新生儿的血清BAP、磷水平降低(P<0.05);3组新生儿在3月龄时VitD_3和IGF-I水平与身长增长、体质量增长速度呈正相关(P<0.05),BAP与身长增长、体质量增长速度呈负相关(P<0.05),6月龄时VitD_3、钙以及IGF-I水平与身长增长、体质量增长速度呈正相关(P<0.05)。结论:骨生化标志物及IGF-I与胎龄、日龄密切相关,VitD_3、BAP、钙及IGF-I与婴儿体格发育密切相关。Objective: To investigate the change of bone biochemical markers and Insulin-like Growth Factor-I( IGF-I) of infants in different gestational ages( 0-6 months after birth),and its correlation with physical indexes. Methods: Based on different gestational ages,109 newborns were divided into early-preterm group( n = 37),late-preterm group( n = 41),and full-term group( n = 31).Then serum levels of Vitmain D3( VitD3),bone alkaline phosphatase( BAP),calcium,phosphorus and IGF-I were detected at the 1 hour,7 days,3 and 6 months of age respectively. At the same time,physical parameters including body length,head circumference and weight were measured. The correlation of serum levels of VitD3,BAP,calcium and phosphorus with physical indexes was analyzed.Results: The levels of serum VitD3 and calcium in all groups and serum IGF-I in the early-preterm group increased with the increase of age( P〈0. 05). The levels of serum BAP and phosphor in all groups decreased with the increase of gestational age and age( P〈0. 05). At 3 months of age,the levels of VitD3 and IGF-I in the infants in all groups were positively correlated with the growth of body length and body mass,while the level of BAP was negatively correlated with the growth of body length and the growth rate of body mass( P〈0. 05). The levels of VitD3,calcium and IGF-I were positively correlated with the growth of body length and body mass at 6 months of age( P〈0. 05). Conclusions:1. The levels of bone biochemical markers and IGF-I are closely related to gestational age and age; they can reflect the status of bone metabolism in infants. 2. VitD3,BAP,calcium and IGF-I are closely associated with physical parameters of infants.
