止痛方与通腑方在阿片类药物治疗癌性疼痛中的临床应用

摘　　要：目的:探讨止痛方与通腑方在阿片类药物治疗癌性疼痛患者中的疗效。方法:纳入癌性疼痛患者100例,采取随机数字表法,将100例患者分为治疗组和对照组,每组50例。两组患者均予阿片类药物治疗,对照组加服乳果糖口服溶液辅助通便,治疗组外敷止痛方与内服通腑方治疗。比较两组患者的止痛效果、生活质量(KPS)及便秘量表评分(CCS)情况。结果:治疗组止痛总有效率(92.0%)高于对照组(76.0%),差异有统计学意义(P<0.05);治疗后,治疗组生活质量疗效及KPS评分均较高于对照组,差异有统计学意义(P<0.01);治疗后,两组便秘量表评分均升高,但治疗组低于对照组,差异有统计学意义(P<0.05)。结论:止痛方外敷联合通腑方内服能有效提高肿瘤患者的镇痛疗效,缓解其便秘程度,改善患者生活质量,减轻毒副反应。Objective： To investigate the effect of Zhitong prescription and Tongfu prescription in the treatment of cancer pain with opioids. Methods： The 100 patients with cancer pain were enrolled in the study and randomly divided into treatment group and control group （n=50）. Patients were treated with opioids in two groups. Patients were treated with Lactulose Oral Solution in control group and Zhitong prescription as well as Tongfu prescription in treatment group. The analgesic effect, quality of life （KPS） and constipation scale score （CCS） were compared between the two groups. Results： The effective rate of analgesia in the treatment group （92.0%） was higher than that in the control group （76.0%）, and the difference was statistically significant （P〈 0.05）. The treatment group showed higher quality of life （QOL） effect and KPS scores than the control group after treatment （P〈0.01）. The constipation scores increased in the two groups after treatment, and the treatment group showed lower constipation scores than the control group （P〈0.05）. Conclusion： The application of Zhitong prescription combined with Tongfu prescription can effectively improve the analgesic effect of patients with tumor, relieve the degree of constipation, improve the quality of life of patients and reduce the side effects.
