摘　　要：目的:探讨扩散加权成像(DWI)对肝细胞肝癌(HCC)Ki-67表达水平的判断价值。方法:回顾性分析56例HCC患者的DWI图像,测量肿瘤表观弥散系数(ADC)值。计算ADC值与Ki-67表达指数的相关性,将HCC患者分为Ki-67高表达(Ki-67≥10%)组(n=40)和低表达(Ki-67<10%)组(n=16)并比较两组的ADC值,应用接受者操作特征(ROC)曲线评价ADC值判断Ki-67表达的诊断性能。结果:ADC值与Ki-67之间具有显著负相关性(rho=-0.504,P=0.0001)。"低Ki-67"组的HCC的ADC值明显高于"高Ki-67"组(1.98×10^(-3)mm^2/s vs.1.57×10^(-3)mm^2/s,P=0.001)。应用ADC值判断低Ki-67表达表达的曲线下面积为0.81。以≥1.89×10^(-3)mm^2/s为界值判断低Ki-67表达的敏感性、特异性、阳性预测值、阴性预测值分别为56.3%、98.2%、90.0%、88.7%。结论:ADC值对HCC的Ki-67表达水平具有潜在预测价值。Purpose： To explore the value of diffusion weighted imaging （DWI） in assessing Ki-67 labeling index （LI） of hepatocellular carcinoma （HCC）. Methods： Fifty-six patients with HCC who underwent DWI were included and apparent diffusion coefficient （ADC） values were measured retrospectively. The correlation between ADC values and Ki-67 LI was evaluated, and ADC values were also compared between low Ki-67 group （n=16, Ki-67 〈 10%） and high Ki-67 group （n=40; Ki-67 〉10%）. Receiver operator characteristics （ROC） analysis was further performed for evaluating the diagnostic performance. Results： Mean ADC of HCC demonstrated significant correlations with Ki-67 LI （rho= -0.504, P=0.0001）. Mean ADC in HCCs with low Ki-67 LI were significantly higher than that in the HCCs with high Ki-67 LI （1.98×10^-3mm^2/s vs. 1.57×10^-3mm^2/s, P=0.001）. Areas under the ROC curve for identification of low Ki-67 LI was 0.81 with ADC cut-off value of 1.89×10^-3mm^2/s and the sensitivity, specificity, positive predictive value and negative predictive value were 56.3%, 98.2%, 90.0%, 88.7%, respectively. Conclusions： ADC value was helpful for predicting Ki-67 LI of HCC.
