摘 要:目的:评价医院-社区一体化管理模式对老年社区高血压患者的防治效果。方法:采用计算机随机抽样从雅安市华兴街社区卫生服务中心慢性病管理网络中选取老年高血压患者220例;随机分两组,观察组采取医院-社区一体化管理模式进行管理,对照组采取单一社区管理模式,不进行特殊干预。随访12个月,评价患者血压控制情况及自我管理能力。结果:观察组血压达标率优于对照组,且患者高血压相关自我管理能力优于对照组,差异有统计学意义(P<0.05)。结论:医院-社区一体化管理模式可提高老年社区高血压患者血压达标率,改善老年高血压患者自我管理能力。Objective:To evaluate the effect of the hospital-community integrated management model on the prevention and treatment of hypertension patients in the elderly community.Methods:220 cases of elderly hypertensive patients were selected from the management of chronic disease network of community health service center of Huaxing street in Ya'an city- by computer random sampling.They were divided into the two groups randomly.The observation group was managed by the integrated management model of the hospital community,and the control group took a single community management model without special intervention.After 12 months of follow-up,the patients' blood pressure control and self-management ability- were evaluated. Results:The compliance rate of blood pressure in the observation group was better than that of the control group,and the self-management ability- of hypertension was superior to the control group,and the difference was statistically significant(P 〈 0.05). Conclusion:The integrated management model of hospital and community can improve the standard rate of blood pressure in the elderly patients with hypertension and improve the self management ability- of the elderly hypertensive patients.
