人感染H7N9禽流感病毒的胸部影像学表现

摘　　要：目的探讨人感染H7N9禽流感病毒性肺炎的影像学表现。方法回顾性分析笔者医院收治的4例确诊为人感染H7N9禽流感病毒性肺炎患者的临床及影像学资料。结果 2例患者行X射线和CT检查,另2例只做CT检查。3例患者呈多灶多肺叶分布,以两肺下叶及背侧最多;4例病灶呈磨玻璃密度影合并大片状实变;2例小叶间隔增厚、2例支气管血管束增粗;4例实变肺组织内见充气支气管征。其他征象:少量胸腔积液3例,未见肺门及纵隔淋巴结肿大。结论人感染H7N9禽流感肺炎具有一定的影像学特征,影像学检查有助于诊断及监测病情变化。Objective To explore the chest imaging manifestations of patients with viral pneumonia of H7 N9 avian influenza.Methods Clinical and imaging data of 4 cases with H7 N9 avian influenza infection in our hospital were analyzed retrospectively.Results Two patients underwent X-ray and CT examination,and another 2 patients only underwent CT examination. Of the 4 patients,3 showed a multifocal and multiple lung distribution in which the largest number were located in the low lobes and dorsal sides in both lungs.The lesions of the 4 cases were grated glass density complicated with large shape consolidation.There was a thickening of lobular septum in 2 cases and a thickening of bronchial blood vessel bundle was found in another 2 cases.The bronchial sign was found in lung tissue of all 4 cases.Other signs included a small amount of pleural effusion in 3 cases but no pulmonary portal and mediastinal lymph node enlargement was observed.Conclusion Human infection with H7 N9 avian influenza has certain imaging characteristics. Imaging studies can help to diagnose and monitor the disease progression.
