保留股骨颈短柄假体THA治疗晚期股骨头坏死的中期效果观察

摘　　要：目的观察保留股骨颈短柄假体全髋关节置换术(THA)治疗晚期股骨头坏死(ONFH)的中期效果。方法选取2008年6月至2011年12月在郑州大学第一附属医院骨科接受C.F.P.假体THA治疗的12例(13髋)晚期ONFH患者,其中10例(11髋)患者获得完整随访。观察患者的双髋关节标准正位片,根据髋臼假体和股骨假体的位置及其周围骨量的变化等对临床效果进行评价。记录术后并发症发生情况,并根据Harris髋关节评分评定术后髋关节功能改善情况。结果 10例患者随访时间为5.1~8.5 a,平均(6.2±0.6)a。终末随访时患者Harris评分[(90.8±6.1)分]较术前[(42.8±8.6)分]提高了(48.0±7.3)分,差异有统计学意义(P<0.05)。随访回示:优8髋,良3髋。术后无并发症发生。结论使用保留股骨颈短柄假体THA治疗晚期ONFH患者的中期效果良好。Objective To investigate the mid-term clinical outcomes on a short-stem prosthesis preserving femoral neck in total hip arthroplasty（ THA） for treating osteonecrosis of the femoral head（ ONFH） in advanced stage. Methods A total of 12 young patients with 13 hips suffering from ONFH in advanced stage were treated by a short-stem prosthersis preserving femoral neck in THA from June of 2008 to December of 2011 in the First Affiliated Hospital of Zhengzhou University. A total of 10 patients with 11 hips were available for this review. The mid-term clinical outcomes were evaluated according to imaging and clinical evaluation. The standard X-ray of the hips was observed and the clinical outcomes were evaluated according to the position of acetabular prosthesis and femoral prosthesis and the changes of surrounding bone mass. The postoperative complications were recored and the improvement of hip function was evaluated according to Harris evaluation score. Results All the patients were followed up for 5. 1 to 8. 5 years,with an average time of（ 6. 2 ± 0. 6） years. The mean Harris hip score improved from preoperation（ 42. 8 ± 8. 6） to（ 92. 8 ± 6. 1） points,with（ 48. 0 ± 7. 3） points improved at the time of final follow-up. The results were excellent in 8 hips and good in 3 hips in 10 patients. There was no complication after operation. Conclusion The mid-term clinical outcome on a short stem preserving femoral neck in THA in the treatment of ONFH in advanced stage is good.
