摘　　要：目的评价射频凝血器在大肝癌切除术中的应用价值。方法选取2015年8月至2017年8月于郑州大学第一附属医院肝胆外科接受常规大肝癌切除术的42例肝癌患者(对照组)和接受射频凝血器辅助下大肝癌切除术的34例肝癌患者(观察组),比较两组患者手术时间、术中出血量、是否阻断第一肝门、术中是否输血以及术后并发症发生情况。结果观察组手术时间短于对照组,术中出血量少于对照组,阻断第一肝门比例和术中输血比例低于对照组,差异有统计学意义(P<0.05)。结论射频凝血器在大肝癌切除术中能有效缩短手术时间,减少术中出血量,降低手术风险,具有较大的应用价值。Objective To evaluate the value of radiofrequency coagulation device in resection of large hepatocellular carcinoma. Methods A total of 42 patients with conventional hepatectomy（ control group） and 34 patients underwent resection of large hepatectomy with radiofrequency coagulation devise（ observation group） from August of 2015 to August of 2017 in the First Affiliated Hospital of Zhengzhou University were enrolled. Operation time,intraoperative blood loss,whether to block the first hepatic portal,intraoperative blood transfusion and postoperative complications were compared between two groups. Results The operation time was shorten in observation group than that in control group,intraoperative blood loss was less in observation group than that in control group,blocking rate of the first portal and intraoperative blood transfusion rate were lower in observation group than these in control group,and the differences were statistically significant（ P 0. 05）. Conclusion Radiofrequency coagulation devise can shorten the operation time,decrease the intraoperative blood loss,and reduce the risk of operation in resection of large hepatocellular carcinoma,which have great application value.
