摘  要:目的探讨Chemerin/ChemR23与糖尿病肾病肾小球血管内皮细胞内皮-间质转分化(EndMT)的相关性。方法糖尿病无肾病患者(DM组)20例,糖尿病肾病患者(DN组)60例,分为3组,A组:尿白蛋白/尿肌酐(UACR)为30~300 mg/g,eGFR≥60 ml/(min·1.73 m^2)的患者20例;B组:UACR>300 mg/g,eGFR≥60 ml/(min·1.73 m^2)的患者20例;C组:UACR>300 mg/g,eGFR<60 ml/(min·1.73 m^2)的患者20例,以同期健康体检者(20例)为对照(NC组),ELISA法检测血清中Chemerin、TGF-β1的分泌量;DN患者中行肾活检术的共8例,以10例正常肾组织标本作对照,免疫组化检测肾组织Chemerin、ChemR23、α-SMA及CD31的表达。结果与NC组相比,DM组及DN组患者血清中Chemerin、TGF-β1的含量增加,且DN组升高更为明显,二者具有正相关性并随病情进展而逐渐增加;肾组织中Chemerin、ChemR23含量随糖尿病肾病病情进展而增加,α-SMA表达增多,CD31表达则减少。结论 Chemerin/ChemR23参与了DN中的EndMT过程。Objective To explore the effect of Chemerin/ChemR23 on endothelial -mesenchymal transition (EndMT) in diabetic nephropathy. Methods Twenty patients of diabetes without nephropathy (DM), 60 patients of diabetic nephropathy (DN), which were subgrouped into 3 groups: group A (n = 20), urinary albumin/urinary creatinine rate (UACR) was 30 -300 mg/g, eGFR≥60ml/(min·1.73 m2) ; group B (n =20), UACR 〉300 mg/g, eGFR≥60 ml/(min·1.73m2) ; group C (n =20), UACR 〉 300 mg/g, eGFR 〈60ml/(min·1.73 m2), normal control group (NC :n = 20), Chemerin and TGF- β1 expression in serum were detected by ELISA method. Furthermore, the protein expression of Chemerin, ChemR23, α - SMA and CD31 in kidney tissues were determined by itrmmnehistochemical method. Results Compared with normal control groups, Chemerin and TGF -β1 expression were increased significantly in sermn of DN patients, and increased with the progress of the disease. Spearman correlational analysis showed that the expression of Chemerin and TGF -β1 were positively correlated. Meanwhile, itrmmnohistochemistry showed that protein expression levels of Chemerin, ChemR23 and α -SMA increased notably with the progression of disease, CD31 descreased. Conclusion Chemerin/ChemR23 play a role in the process of EndMT in DN patients.
