升清降浊胶囊治疗慢性肾脏病4-5期伴高尿酸血症患者的疗效观察

摘　　要：【目的】观察升清降浊胶囊对慢性肾脏病(CKD)4-5期伴高尿酸血症(HUA)患者的降尿酸作用。【方法】将80例CKD4-5期伴HUA患者随机分为治疗组和对照组,每组各40例。对照组给予西医常规一般治疗,治疗组在西医常规一般治疗的基础上加服升清降浊胶囊。观察2组患者治疗前后的血尿酸水平及肾功能[血肌酐(SCr)、内生肌酐清除率(CCr)]的变化情况。【结果】(1)治疗后,2组患者血尿酸浓度均较治疗前有所下降,对照组下降了7.65%,但差异无统计学意义(P>0.05),治疗组下降了21.53%,差异有统计学意义(P<0.01),治疗组在降低血尿酸浓度方面明显优于对照组(P<0.01)。(2)经治疗后,治疗组HUA总有效率为70.0%,明显高于对照组的47.5%,组间比较(χ2检验),差异有统计学意义(P<0.05);经Mann-Whitney U检验,治疗组的总体疗效也明显优于对照组,差异有统计学意义(P<0.05)。(3)治疗后,2组患者的SCr均显著下降而CCr均显著上升(P<0.05)。其中,治疗组患者的SCr浓度较治疗前下降了21.88%,高于对照组的10.48%,差异有统计学意义(P<0.01);CCr上升了46.99%,高于对照组的17.55%,差异有统计学意义(P<0.01)。【结论】升清降浊胶囊能显著降低CKD4-5期伴HUA患者的血尿酸水平,改善肾脏功能,对CKD4-5期患者HUA有治疗作用。Objective To Observe the efficacy of Shengqing Jiangzhuo Capsules（SJC）on decreasing the uric acidin chronic kidney disease patients at stage 4-5（CKD4-5）complicated with hyperuricemia（HUA）. Methods Eighty cases of CKD4-5 complicated with HUA were randomly divided into treatment group and control group,40 cases in each group. The two groups were given conventional western medicine treatment, and additionally thetreatment group was treated with SJC orally. The blood uric acid level, serum creatinine（SCr）concentration andendogenous creatinine clearance（CCr）of the two groups before and after treatment were observed. Results（1）Aftertreatment, blood uric acid level was reduced in the two groups,and the decrease rate in the control group was7.65%（P〈0.05）and that in the treatment group was 21.53%（P〈0.01）. The treatment group had better effect ondecreasing uric acid than the control group（P〈0.01）.（2）After treatment,the total effective rate for HUA intreatment group was 70.0%,which was higher than 47.5% in the control group（χ^2 test,P〈0.05）.The results ofMann-Whitney U test showed that the overall therapeutic efficacy of the treatment group was superior to that of thecontrol group（P〈0.05）.（3）After treatment,SCr concentration was decreased while CCr was increased in the twogroups（P〈0.05 compared with those before treatment）. SCr concentration was reduced by 21.88% in the treatmentgroup and was reduced by 10.48% in control group（P〈0.01）,and CCr was increased by 46.99% and 17.55% inthe treatment group and the control group respectively（P〈0.01）. Conclusion SJC can significantly reduce theblood uric acid level and improve renal function of CKD4-5 patients complicated with HUA,which is effective fortreating CKD4-5 complicated with HUA.
