摘 要:目的了解和分析鲍曼不动杆菌的临床分布及细菌耐药情况,为临床合理使用抗菌药物、控制医院感染提供依据。方法回顾性分析本院2014~2016年临床分析出的1 663株鲍曼不动杆菌的临床科室分布、标本来源以及耐药情况。结果共检出鲍曼不动杆菌1 663株,标本检出最高的是痰1 294株,占77.8%,其次分别为尿液86株(5.2%)、肺泡灌洗液82株(4.9%);科室分布主要在重症医学科717株,占43.1%,其次分别在神经外科164株(9.9%)和呼吸内科155株(9.3%);鲍曼不动杆菌对氨苄西林、氨苄西林/舒巴坦和氨曲南的耐药率高达90%以上;对其他药物耐药率均大于60%,耐药率最低的为头孢哌酮/舒巴坦,8.3%。结论鲍曼不动杆菌是医院感染重要的条件致病菌,本院此菌对常用抗菌药物耐药广泛,且耐药率较高,应加强对其耐药性的监测,指导临床正确且合理使用抗菌药物,预防与控制耐药菌株的流行及医院感染。Objective To understand and analyze the clinical distribution and bacterial resistance of Acinetobacter baumannii, provides basis for rational use of antimicrobial agents and control of hospital infection. Methods Retrospective analysis of the distribution, source and drug resistance of the 1 663 strains of the acinetobacter baumannii in our hospital from 2014 to 2016. Results A total of 1 663 strains of acinetobacter baumannii were detected.the samples showed the highest phlegm, 1 294 strains, 77.8 %, followed by 86(5.2%) urine and 82(4.9 %) of alveolar lavage; The distribution of the department was mainly in the 717 strains of severe medicine, accounting for 43.1%, followed by 164 strains of neurosurgery(9.9%)and respiratory medicine 155(9.3%). The drug resistance rate of acinetobacter baumannii was more than 90% in ampicillin, ampicillin/sulbatan and aminotram. The drug resistance rate of other drugs was greater than 60%, the lowest drug resistance rate was cefoperazone/sulbatan, 8.3%. Conclusion Acinetobacter baumannii is an important condition for the infection of the hospital. The bacteria in our hospital are widely used for antibacterial drug resistance and high tolerance rate, should strengthen the monitoring of its resistance, guide the proper and rational use of antimicrobial agents,prevent and control the epidemic of drug-resistant strains and hospital infection.
