腹腔镜在胃十二指肠溃疡穿孔修补术中的应用价值

摘　　要：目的探讨腹腔镜在胃十二指肠溃疡穿孔修补术中的应用价值。方法对80例胃十二指肠溃疡穿孔患者的临床资料进行统计分析,依据手术术式将这些患者分为腹腔镜胃十二指肠溃疡穿孔修补术组(腹腔镜组,n=40)和开腹胃十二指肠溃疡穿孔修补术组(开腹组,n=40)两组,对两组患者的术中术后指标、术后并发症发生情况及镇痛药物应用情况进行统计分析。结果腹腔镜组患者的术后开始下床时间、肠道恢复通气时间、住院时间均显著短于开腹组(P<0.05);术中出血量显著少于开腹组(P<0.05);腹腔镜组术后并发症发生率、镇痛药物应用率分别为5.0%(2/40)、20.0%(8/40)均显著低于开腹组17.5%(7/40)、52.5%(21/40)(P<0.05)。结论腹腔镜在胃十二指肠溃疡穿孔修补术中的应用价值较开腹高。Objective To explore the value of laparoscopy in the repair of gastroduodenal ulcer perforation. Methods The clinical data of 80 patients with gastroduodenal ulcer perforation treated from April 2015 to April 2017 were analyzed statistically. According to the surgical procedure,these patients were divided into laparoscopic gastroscope（Laparoscopic group, n=40） and open stomach duodenal ulcer perforation repair group（open group, n=40） two groups, the two groups of patients with postoperative indicators, Postoperative complications and the application of analgesic drugs for statistical analysis. Results The time of getting out of bed in the laparoscopic group was significantly shorter than that in the open group（P〈0.05）. The intraoperative blood loss was significantly lower than that in the open group（P〈0.05）. The incidence of postoperative complications and analgesic drugs in the laparoscopic group were 5.0%（2/40） and 20.0%（8/40）, both significantly lower than that of the open group 17.5%（7/40）and 52.5%（21/40）（P〈0.05）. Conclusion The application value of laparoscopy in the repair of gastroduodenal ulcer perforation is higher than that of laparotomy.
