锁骨下动脉盗血综合征介入治疗的回顾性分析

摘　　要：目的评价介入治疗锁骨下动脉盗血综合征(SSS)患者的安全性和有效性。方法回顾性分析该科2011—2017年收治的19例SSS患者的临床资料、手术方式、手术路径、围手术期并发症、早期疗效及随访远期通畅率等。结果 19例SSS患者中18例成功放入支架,12例经股动脉路径行手术,7例联合肱动脉路径行手术,2例左锁骨下动脉闭塞患者术中放置椎动脉保护伞。围手术期无1例死亡,1例术中出现心率一过性下降,1例术中发生锁骨下动脉夹层,1例术后穿刺处渗血,18例患者术后上肢缺血症状、盗血症状较术前明显改善。19例患者术后均获得随访,其中1例患者术后9个月出现支架内再狭窄。18例支架植入患者术后1、3、5年支架累积通畅率分别为94.44%、83.33%、77.78%。结论血管腔内治疗SSS是一种安全有效的治疗方式,值得临床首选推荐。Objective To assess the safety and efficacy of interventional treatment of patients with subclavian steal syndrome（SSS）.Methods The clinical data,surgical methods,surgical pathways,perioperative complications,early results and follow-up long-term patency rates of 19 SSS patients admitted to this department from 2011 to 2017 were retrospectivelyanalyzed.Results 18 cases were successfully placed in the stent,12 cases were through the femoral artery,7 cases were associated with brachial artery,2 cases with left subclavian artery occlusion were placed intraoperatively with vertebral artery protection umbrella.There were no death in the perioperative period,1 case had transient decrease in heart rate during the operation,1 case had subclavian artery dissection duringthe operation,1 case had bleeding at the postoperative puncture site,18 cases of postoperative upper limb ischemia or steal symptoms were significantly improved compared with preoperative. 19 pa-tients were followed up postoperatively,1 patient had in-stent restenosis 9 months after surgery.The cumulative patency rates of stent in 18 cases of stent implantation at 1 year,3 years,5 years after operation were 94.44%,83.33% and 77.78%,respectively.Conclusion Endovascular treatment of SSS is a safe and effective treatment,the clinical recommendation should be the preferred use.
