鼻窦内窥镜术联合缓冲高渗盐溶液鼻腔冲洗治疗变应性鼻—鼻窦炎的疗效观察

摘　　要：目的 探讨功能性鼻窦内窥镜手术(FESS)联合缓冲高渗盐溶液鼻腔冲洗治疗变应性鼻—鼻窦炎(ARS)的疗效。方法 选择我院于2015年3月至2017年5月收治的ARS患者86例为研究对象,采用数表法将其随机分为观察组(43例)和对照组(43例),两组均采用FESS治疗,观察组术后采用缓冲高渗盐溶液冲洗鼻腔,对照组术后采用生理盐水冲洗鼻腔,两组均治疗3个月,对比两组临床有效率、治疗前后临床症状评分及血清炎症因子水平变化情况。结果 观察组临床总有效率为95.35%(41/43),高于对照组的69.77%(30/43),差异有统计学意义(P<0.05);治疗后观察组患者临床症状评分低于对照组(P<0.05);与对照组相比,观察组血清白细胞介素-6(IL-6)、白细胞介素-8(IL-8)水平降低,血清白细胞介素-10(IL-10)水平则升高,差异均有统计学意义(P<0.05)。结论 FESS术后采用缓冲高渗盐溶液进行鼻腔冲洗治疗ARS可显著提高临床疗效,改善患者临床症状及炎性反应。Objective To explore the clinical effect of endoscopic sinus surgery（FESS） combined with buffered hypertonic saline solution for nasal irrigation in patients with allergic rhinitis and sinusitis（ARS）.Methods 86 patients with ARS who were treated in our hospital from March 2015 to May 2017 were selected as the subjects, and the patients were divided into observation group（43 cases） and control group（43 cases） by random number table.Both the two groups were treated with FESS.The observation group were given buffered hypertonic saline solution for nasal irrigation, while the control group were given normal saline.The two groups were treated for 3 months.Then, the clinical effective rate, clinical symptom score and serum inflammatory factor level were compared between the two groups.Results The total effective rate of the observation group was 95.35%（41/43）, which was higher than 69.77%（30/43） of the control group, with statistically significant difference（ P0.05）.The clinical symptom score of the observation group after treatment was lower than that of the control group（P 0.05）.The serum levels of interleukin-6（IL-6） and interleukin-8（IL-8） of the observation group were decreased when compared with the control group, and the serum levels of interleukin-10（IL-10） were increased.The differences were statistically significant（P0.05）.Conclusion ARS treatment with buffered hypertonic saline solution for nasal lavage after FESS can significantly improve the clinical efficacy and improve the clinical symptoms and inflammatory factors.
