摘 要:目的观察加味温氏奔豚汤联合丹参饮治疗慢性阻塞性肺疾病急性加重(AECOPD)合并慢性肺心病急性期患者的临床疗效。方法:AECOPD合并慢性肺心病急性期患者82例,采用随机数表法分组,各41例。对照组予以常规治疗,观察组在对照组基础上予以加味温氏奔豚汤联合丹参饮治疗。对比2组治疗效果,检测对比治疗前后2组肺功能[第1秒用力呼气容积(FEV1)、FEV1占预计值]、血液流变学(红细胞压积、血液黏度、血小板凝聚率、纤维蛋白原)。结果观察组治疗总有效率97.56%(40/41)高于对照组80.49%(33/41),差异有统计学意义(P<0.05);治疗后观察组FEV1、FEV1/预计值水平均高于对照组,差异有统计学意义(P<0.05);治疗后观察组红细胞压积、血液黏度、血小板凝聚率、纤维蛋白原水平均低于对照组,差异有统计学意义(P<0.05)。结论加味温氏奔豚汤联合丹参饮治疗AECOPD合并慢性肺心病急性期患者,可提高临床疗效,增强患者肺功能,改善血流动力学。Objective: To observe the clinical efficacy of Modified Wenshi Benbai decoction combined with Danshen decoction in the treatment of acute exacerbation of chronic obstructive pulmonary disease(AECOPD) combined with chronic pulmonary heart disease. Methods: Eighty-two patients with AECOPD in the acute phase of chronic cor pulmonale were grouped using a random number table method, 41 in each case. The control group was given routine treatment.The observation group was treated with Modified Wenshi Benbai decoction and Danshen decoction on the basis of the control group. Comparing the therapeutic effects of the two groups, the two groups of lung function before and after the treatment were examined [forced expiratory volume in 1 second(FEV1), FEV1 predicated value], hemorheology(hematocrit, blood viscosity, platelet aggregation rate, fibrin) original). Results: The total effective rate in the observation group was 97.56%(40/41) higher than that in the control group(80.49%(33/41)). The difference was statistically significant(P〈0.05). After treatment, the FEV1 and FEV1/predicted values in the observation group were higher than those in the control group, and the difference was statistically significant(P〈0.05). After treatment, hematocrit, blood viscosity, platelet aggregation rate, and fibrinogen level in the observation group were lower than those in the control group, and the difference was statistically significant(P〈0.05). Conclusion: Modified Wenshi Benbai decoction combined with Danshen decoction in the treatment of AECOPD patients with chronic pulmonary heart disease can improve clinical efficacy, enhance lung function of patients, and improve hemodynamics.
