针刺配合微波治疗肩关节周围炎的疗效观察

摘　　要：目的观察针刺配合微波治疗肩关节周围炎的临床疗效及对三角肌表面肌电信号的影响。方法将120例肩关节周围炎患者随机分为治疗组和对照组,每组60例。治疗组采用针刺肩髃、肩前、肩髎、臂臑穴配合微波治疗,对照组采用单纯针刺治疗。两组患者每个疗程最后一次治疗分别采用肌电仪记录各穴针刺前后及行针5 s时三角肌的表面肌电信号(sEMG),并比较两组临床疗效。结果两组各疗程肩髃穴行针时及出针后三角肌sEMG与同组针刺前比较,差异均具有统计学意义(P<0.05)。治疗组各疗程出针后及各穴行针时三角肌sEMG与对照组比较,差异均具有统计学意义(P<0.05)。治疗组总有效率和治愈率分别为95.0%和23.3%,对照组分别为85.0%和15.0%,两组比较差异均具有统计学意义(P<0.05)。结论针刺配合微波是一种治疗肩关节周围炎的有效方法。Objective To observe the clinical efficacy of acupuncture plus microwave in treating scapulohumeral periarthritis and its effect on the surface electromyography（sEMG） of the deltoid muscle. Method One hundred and twenty patients with scapulohumeral periarthritis were randomized into a treatment group and a control group, with 60 cases in each group. The treatment group was intervened by acupuncture at Jianyu（LI15）, Jianqian（extra）, Jianliao（TE14） and Binao（LI14） plus microwave, while the control group was intervened by acupuncture alone. In the final session of each treatment course, before and after needling each point and at 5 s during needling manipulation, the sEMG of the deltoid muscle was recorded by an electromyography apparatus, and the clinical efficacies were compared between the two groups. Result The readings of sEMG of the deltoid muscle measured during needling manipulation for Jianyu（LI15） and after needle removal were significantly different from the reading before acupuncture in both groups（P0.05）. There were significant differences in the readings of sEMG of the deltoid muscle after needle removal and during needling manipulation in each course（P0.05）. The total effective rate and recovery rate were respectively 95.0% and 23.3% in the treatment group, versus 85.0% and 15.0% in the control group, and the between-group differences were statistically significant（P0.05）. Conclusion Acupuncture plus microwave is an effective approach in treating scapulohumeral periarthritis.
